## Executive GIST (TL;DR)
- This 2026 American College of Cardiology scientific statement reviews the current evidence for **antiplatelet** therapy in atherosclerotic cardiovascular disease (ASCVD), focusing on contemporary practice and evolving recommendations. - **Aspirin** is identified as the most widely used antiplatelet agent, but recent data do not support routine aspirin for primary prevention in unselected populations; its use should be reserved for patients at highest ischemic risk. - Advances in stent technology (including drug-eluting stents) and antiplatelet agents have changed evidence-based recommendations for the regimen and duration of **dual antiplatelet therapy (DAPT)**. - Antiplatelet medications remain effective tools to prevent ischemic events, especially in patients with **acute coronary syndrome (ACS)**, but clinicians must weigh ischemic benefit against **bleeding risk** on an individual basis. - The statement compares guideline-based recommendations, summarizes pivotal trials, and addresses antiplatelet management in patients with elevated bleeding risk and those undergoing surgery. - It discusses antiplatelet strategies across different ASCVD phenotypes and in patients who require concurrent antithrombotic therapies, emphasizing individualized decision making. - The statement also covers long-term monitoring, adherence considerations, and practical aspects of antiplatelet use in clinical practice. - Publication details: J Am Coll Cardiol. 2026 Sep 15;88(11):1284-1313. Aspirin remains the most widely used antiplatelet drug. Recent data do not support a benefit for primary prevention of ASCVD in unselected populations with routine aspirin use; consideration should be reserved for those at highest ischemic risk. Similarly, the evolution of drug-eluting stents, antiplatelet medications, and research on the optimal regimen and duration of dual antiplatelet therapy have altered evidence-based recommendations for use of antiplatelets in managing patients diagnosed with ASCVD. Antiplatelet medications remain powerful tools to prevent ischemic events, particularly for those who have experienced an acute coronary syndrome, but the balance between the risk of ischemia and the risk of bleeding poses a perpetual challenge and requires individualized treatment. This scientific statement provides a detailed overview of current evidence for the use of antiplatelets in the management of ASCVD. This includes a comparison of current clinical practice guideline-based recommendations, summary of pivotal trials, and discussion of antiplatelet use in patients with elevated bleeding risk or those undergoing surgery. Considerations for use of antiplatelets in those with different phenotypes of ASCVD and those with indications for concurrent use of antithrombotic medications, as well as long-term monitoring and adherence, are addressed. **Keywords:** ACC Scientific Statement; acute coronary syndrome; aspirin; atherosclerotic cardiovascular disease; bleeding risk; coronary artery disease; dual antiplatelet therapy; percutaneous coronary intervention; prevention.
