## Executive GIST (TL;DR)
- This systematic review and meta-analysis assessed the association between **preoperative anaemia** and postoperative outcomes in adults undergoing cardiac surgery using PRISMA 2020 methods. - Searches covered five databases for peer-reviewed English-language studies published from 2017 to July 2026. - Fourteen observational cohort studies were included, comprising 74,571 adults; 17,349 (23.3%) had preoperative anaemia. - Random-effects meta-analyses used restricted maximum likelihood estimation and Hartung-Knapp confidence intervals. Crude exact count estimates were pooled primarily; adjusted estimates were analysed separately. - Preoperative anaemia was associated with increased odds of postoperative **red cell transfusion** (OR 3.66; 95% CI 2.14–6.24). - Anaemia was linked to higher odds of **acute kidney injury or renal failure** (OR 2.43; 95% CI 1.84–3.21). Certainty for renal outcomes was rated low. - Anaemia was associated with greater odds of early mortality (OR 2.11; 95% CI 1.40–3.17) and postoperative infection (OR 1.49; 95% CI 1.21–1.82). Certainty for these outcomes was very low. - No statistically significant association was found between preoperative anaemia and postoperative stroke (OR 1.16; 95% CI 0.43–3.17). - The review concludes preoperative anaemia is an important perioperative risk marker—particularly for transfusion and renal complications—but causality cannot be inferred from the included observational data. - The authors declared no competing interests and reported that no new individual-level data were generated.

This systematic review and meta-analysis synthesised contemporary observational evidence in accordance with PRISMA 2020. Five databases were searched for peer-reviewed English language studies published from 2017 to July 2026. Random effects meta-analyses used restricted maximum likelihood estimation with Hartung-Knapp confidence intervals; crude exact count estimates were synthesised primarily and adjusted estimates separately. Fourteen observational cohort studies involving 74,571 adults, including 17,349 (23.3%) with anaemia, were included. Preoperative anaemia was associated with red cell transfusion (OR, 3.66; 95% CI, 2.14-6.24), acute kidney injury or renal failure (OR, 2.43; 95% CI, 1.84-3.21), early mortality (OR, 2.11; 95% CI, 1.40-3.17), and postoperative infection (OR, 1.49; 95% CI, 1.21-1.82), but not postoperative stroke (OR, 1.16; 95% CI, 0.43-3.17). Certainty was low for renal outcomes and very low for the other outcomes. Preoperative anaemia is an important perioperative risk marker, particularly for transfusion and renal complications, although causality cannot be established. ### Competing Interest Statement The authors have declared no competing interest. ### Clinical Protocols ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: This systematic review used only publicly available aggregate data reported in peer-reviewed articles identified through PubMed, Scopus, Embase, Web of Science, and the Cochrane Library. The original studies are fully cited in the manuscript and can be located through their references and database records. No individual-level patient data were accessed or collected. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable. Yes ## Data Availability No new data were generated or collected in this study. All data were extracted from publicly available aggregate data reported in peer-reviewed articles. The included studies are fully cited in the manuscript and can be located through their reference lists and the databases searched, including PubMed, Scopus, Embase, Web of Science, and the Cochrane Library.
