## Executive GIST (TL;DR)
- The study tracked serum **GDF-15** in patients with **non-ST-elevation acute coronary syndrome (NSTE-ACS)** at admission, and at 1 and 3 hours thereafter, to evaluate early temporal changes. - Investigators measured GDF-15 alongside **NT-proBNP** and **hs-cTnT** and used Spearman correlations and linear regression to explore associations with admission levels and short-term changes. - The cohort showed a slight upward trend in **GDF-15** within the first 3 hours; the 1-hour change correlated with contemporaneous **hs-cTnT** changes. - Patients with an increased 1-hour GDF-15 versus admission had higher recurrent ischemic event rates at 1 year (1.9% vs 17.5%, P = 0.019), per the authors' report. - In Cox models, the 1-hour rise in GDF-15 remained an independent predictor after adjustment (adjusted HR 1.71, 1.28 to 228.75; P = 0.03), according to the preprint. - The study team reported that serial GDF-15 provided incremental prognostic information beyond traditional risk factors and biomarkers, assessed by AUC comparisons. - Serum sampling schedule, assay details, sample size, and exact model covariates were not fully described in the abstract; the full preprint should be consulted for methodology specifics. - The research was approved by the Beijing Anzhen Hospital Ethics Committee (2021-072), funder declared, and data are available from the corresponding author on request. The paper is a medRxiv preprint and not peer-reviewed.

Our study evaluated the temporal evolution of GDF-15 in the acute phase and its association with recurrent ischemic events. Methods: Serum samples were collected from patients with NSTE-ACS at admission and at 1 and 3 hours after admission. GDF-15 levels, along with other prognostic biomarkers, NT-proBNP and hs-cTnT, were measured. Spearman rank-correlation coefficients and linear regression models were used to identify the variables associated with GDF-15 levels at admission and with their temporal changes. Associations between GDF-15 and clinical outcomes were evaluated using Cox proportional hazards models. To assess incremental prognostic value, the areas under the curve (AUC) were calculated for each model. Results: GDF-15 levels showed a slight increasing trend within 3 hours of admission. The 1-hour change in GDF-15 was correlated with hs-cTnT changes. Increased 1-hour GDF-15 levels, compared with the admission levels, were associated with a higher risk of recurrent ischemic events during the 1-year follow-up (1.9% vs. 17.5%, P =0.019) and were an independent predictor after adjusting for established clinical variables and biomarkers (adjusted hazard ratio, 1.71 (1.28 to 228.75), P =0.03). Furthermore, serial measurements of GDF-15 provided additional prognostic information beyond traditional risk factors. Conclusions: Serial measurements of GDF-15 of NSTE-ACS patients provided independent and incremental prognostic value for predicting recurrent ischemic events. These findings support the potential utility of GDF-15 as a dynamic biomarker for early risk stratification in the acute management of NSTE-ACS. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
