# Bicytopenia in Adults: Etiology, Hematologic Patterns, and Clinical Profile from a Tertiary Centre

Early identification of the underlying etiology is essential for appropriate management. **Aims:** To study the etiological spectrum, hematological profile, clinical presentation, and patterns of bicytopenia among patients attending a tertiary care Center. **Materials and methods:** The study analyzed the clinical and hematological profile of 110 patients with bicytopenia. Patients were categorized based on age, gender, hematological parameters, etiologies, symptoms, and type of bicytopenia. Statistical analysis was performed using the Chi-square test. **Results:** Patients in the study had a mean age of 41.5 ± 15.42 years with female predominance (54.5%). Viral fever, including dengue, was the leading cause (23.6%), subsequently nutritional deficiency (13.6%) and sepsis (11.8%). Leukopenia with thrombocytopenia was the most common type of bicytopenia (37.3%). Fever was the predominant presenting symptom (48.2%). The type of bicytopenia showed a significant association with of underlying etiology (P < 0.0001). **Conclusion:** Bicytopenia is associated with diverse etiologies ranging from transient infections to serious hematological disorders. Infectious diseases and nutritional deficiencies constituted the major causes in the present study. Recognition of hematological patterns along with systematic clinical evaluation can aid in early diagnosis and timely management. **Résumé** **Contexte:** La bicytopénie se définit par une diminution de deux des trois principales lignées cellulaires sanguines. Elle peut être secondaire à diverses affections infectieuses, carentielles, hématologiques ou systémiques. L’identification précoce de sa cause sous-jacente est essentielle pour instaurer une prise en charge appropriée.**Objectif:** Évaluer le spectre étiologique, le profil hématologique, les manifestations cliniques et les différents types de bicytopénie chez les patients pris en charge dans un centre de soins tertiaires.**Matériels et Méthodes:** Cette étude observationnelle a porté sur 110 patients présentant une bicytopénie. Les patients ont été évalués en fonction de leur âge, de leur sexe, des paramètres hématologiques, de l’étiologie, des manifestations cliniques et du type de bicytopénie. Les données ont été analysées statistiquement à l’aide du test du χ2.**Résultats:** L’âge moyen des patients était de 41,5 ± 15,42 ans, avec une légère prédominance féminine (54,5 %). Les infections virales, notamment la dengue, constituaient la principale cause de bicytopénie (23,6 %), suivies des carences nutritionnelles (13,6 %) et du sepsis (11,8 %). La leucopénie associée à une thrombopénie représentait le type de bicytopénie le plus fréquent (37,3 %). La fièvre était la manifestation clinique prédominante (48,2 %). Une association statistiquement significative a été observée entre le type de bicytopénie et l’étiologie sous-jacente (P < 0,0001).**Conclusion:** La bicytopénie présente un large spectre étiologique, allant d’infections transitoires à des affections hématologiques potentiellement graves. Dans notre population, les infections, en particulier les infections virales, et les carences nutritionnelles constituaient les principales causes.
