## Executive GIST (TL;DR)
- The 2026 expert consensus from Chinese radiation oncology bodies updates guidance on **stereotactic body radiation therapy (SBRT)/stereotactic ablative radiotherapy (SABR)** for early-stage non‑small cell lung cancer (NSCLC). It targets patients who are inoperable or decline surgery. - The document summarizes the technology's development and key research findings to date and integrates domestic and international evidence with clinical experience. - Five core areas are addressed: historical and research review; standards for equipment, treatment planning, and quality assurance; patient selection, treatment workflow, and efficacy evaluation; combining SBRT/SABR with **immunotherapy/targeted therapy** including safety management; and management of complex cases such as **central lung cancer**. - The consensus aims to standardize clinical practice, improve homogeneity of care, facilitate collaborative research, and promote high‑quality implementation of SBRT/SABR in China. - The publication is a consensus statement in Zhonghua Zhong Liu Za Zhi (2026 Sep 23;48(9):1108–1123) with PMID 42764217 and DOI 10.3760/cma.j.cn112152-20260313-00129. - Corporate authors include the Radiation Oncology Branch of the Chinese Medical Association, CSCO Radiation Therapy Expert Committee, Radiation Therapy Committee of China Anti‑Cancer Association, and Radiation Oncologist Branch of the Chinese Medical Doctor Association. - The abstract notes expanded clinical applications driven by radiotherapy technology advances and research; the consensus provides recommendations across equipment, planning, workflow, evaluation, combination therapy, safety, and special scenarios. - All authors declare no conflicts of interest. - The abstract and article are in Chinese; an English abstract is available on PubMed. - Specific operational details such as exact dose/fractionation schedules, numeric quality‑assurance thresholds, or stepwise protocols were not reported in the PubMed abstract and would require consulting the full text for precise parameters.

With advances in radiotherapy technology and in-depth clinical research, its clinical applications have been continuously expanding. To standardize clinical practice, improve treatment homogenization, and facilitate collaborative research, the expert panel developed this consensus based on domestic and international evidence and practical experience. This consensus covers five core components: (1) reviewing the developmental history and key research outcomes of SBRT/SABR; (2) defining standards for equipment configuration, treatment planning, and quality assurance; (3) refining patient selection, treatment workflow, and efficacy evaluation; (4) discussing strategies and safety management of SBRT/SABR combined with immunotherapy/targeted therapy; (5) providing recommendations for managing complex scenarios such as central lung cancer. This consensus aims to provide clinicians with standardized guidance, optimize treatment decisions, enhance patient benefits, and promote the high-quality development of SBRT/SABR in China, ultimately benefiting a broader population of patients with lung cancer. .
