# Emergency-granulopoiesis but not CD4/NK trajectory reproducibly correlates with sepsis mortality

## Executive GIST (TL;DR)
- The authors tested whether early changes over time in blood immune-cell transcriptional programmes predict sepsis mortality more robustly than single baseline measurements, using pre-defined module scores applied to public longitudinal transcriptomic cohorts. - Five immune modules were scored in three discovery whole-blood microarray cohorts; two cohorts had mortality data (82 patients, 24 deaths) used for a pre-specified logistic model of change per 24 hours and inverse-variance fixed-effect meta-analysis with multiple-testing control. - No association in the discovery stage survived correction for multiple testing. The top two directional signals were a rising **CD4/NK** lymphocyte trajectory linked to lower mortality (pooled OR 0.53, 95% CI 0.31–0.90) and a rising **emergency-granulopoiesis** trajectory linked to higher mortality (pooled OR 1.60, 95% CI 0.92–2.79), both per 1 SD change. - The authors performed independent testing on an external transcriptomic validation cohort with serial samples (63 patients, 15 deaths) scored identically, and triangulated findings using clinical-cell-count analogues in MIMIC-IV (12,607 adults meeting Sepsis-3 criteria; 744–4,206 had the required serial measurements per analogue). - Independent testing reversed the discovery ordering: the **emergency-granulopoiesis** trajectory reproduced directionally and with similar effect size though not singly reaching conventional significance in the transcriptomic validation (OR 1.72, 0.92–3.20; p = 0.088); pooled estimate became 1.65 (1.09–2.50). - The emergency-granulopoiesis signal was positive across nine pre-specified sensitivity analyses and was supported by two of three clinical analogues, including the **neutrophil-to-lymphocyte ratio** (OR 1.31, 1.20–1.42). - The **CD4/NK** discovery association failed independent reproduction (validation OR 1.06, 0.59–1.89), showed a null effect in its most favorable temporal window, and lacked support from a well-powered clinical analogue. - The study used only existing de-identified datasets (GEO accessions GSE95233, GSE54514, GSE57065; ArrayExpress E-MTAB-5273; MIMIC-IV v3.1) and archived frozen per-patient exposure and module-score tables; code and derived data are publicly available. - Authors report no competing interests; as a secondary analysis of public/de-identified data, no new ethics approvals were sought or required by the authors, and MIMIC-IV access followed credentialing with an IRB waiver for resource creation.

We scored five immune modules, frozen before analysis, in three public longitudinal whole-blood microarray sepsis cohorts and fitted a logistic model ladder fixed in advance to the change per 24 hours in the two cohorts with mortality data (82 patients, 24 deaths), pooling by inverse-variance fixed-effect meta-analysis with Benjamini-Hochberg control. No association survived correction for multiple testing. The two leading signals were a rising CD4/NK lymphocyte trajectory associated with lower mortality (pooled odds ratio 0.53, 95% confidence interval 0.31 to 0.90) and a rising emergency-granulopoiesis trajectory associated with higher mortality (1.60, 0.92 to 2.79), both per one standard deviation. We then tested both in an independent transcriptomic cohort with serial sampling (63 patients, 15 deaths), scored by the identical frozen method, and against their cell-count analogues in an intensive-care database of 12,607 adults meeting Sepsis-3 criteria, of whom 744 to 4,206 had the serial measurements each analogue required. Independent testing separated the two signals, in the order opposite to the one discovery had suggested. The emergency-granulopoiesis association was reproduced in direction and effect size without reaching conventional significance on its own (validation odds ratio 1.72, 0.92 to 3.20, p=0.088; pooled 1.65, 1.09 to 2.50), was positive in all nine sensitivity analyses, each fixed before the estimates were examined, and was supported by two of its three analogues, including the neutrophil-to-lymphocyte ratio (1.31, 1.20 to 1.42). The CD4/NK association did not reproduce (1.06, 0.59 to 1.89), was null in the window most favourable to it, and received no support from an analogue well powered to detect the discovery effect. The transcriptomic cohorts (GSE95233, GSE54514, GSE57065 from the Gene Expression Omnibus E-MTAB-5273 from ArrayExpress/BioStudies) are publicly available and de-identified, and the original studies that generated them were conducted with the ethical approvals and participant consent reported in their source publications. The cross-modality analyses used MIMIC-IV v3.1, whose creation as a research resource was reviewed by the Institutional Review Board of the Beth Israel Deaconess Medical Center, which granted a waiver of informed consent access was obtained through the PhysioNet credentialing process including human-subjects research training. No participant-identifiable information was accessed at any stage.
