## Executive GIST (TL;DR)
- The study analyzed emergency kidney care admissions at a tertiary public teaching hospital in South India over a 3-year period using case records. - Of 4,700 emergency renal admissions, 3,700 case records were available for analysis. - Over half of admissions were **dialysis** patients; the predominant emergency presentations were **fluid overload** (pulmonary edema), **sepsis**, and catheter-related infections. - Among chronic kidney disease (CKD) patients on dialysis, the most common reasons for emergency care were **fluid overload** (40%) and **sepsis** (21%). - In patients with acute kidney injury (AKI), leading causes of emergency admission were **sepsis** (25%) and obstructive uropathy (11%). - The authors note an increasing burden of CKD and a lack of structured emergency dialysis services at primary and secondary care levels, driving patients toward tertiary centers. - Acute kidney injury related to **sepsis**, poisoning, and obstruction remains a major contributor to emergency renal care. - A higher **SOFA** score at admission was associated with increased mortality; the exact mortality rate and SOFA thresholds were not provided in the abstract. - Aim: to describe patterns and outcomes of emergency kidney care admissions in a tertiary care setting. - Methods: retrospective case record review over 3 years in a tertiary public teaching hospital; further methodological details (exact years, inclusion/exclusion criteria, and statistical methods) were not reported in the abstract.

Non-availability of structured emergency dialysis services at primary/secondary health care levels is the reason why a majority of patients seek emergency care in tertiary care settings. In addition, acute kidney injury (AKI) due to sepsis, poisoning, and obstruction still remains one of the major causes of emergency kidney care services. **Aim:** Our aim was to analyze the patterns and outcomes of emergency kidney care admissions in a tertiary care setting. **Materials and methods:** A case record study of patients admitted over a duration of 3 years to the emergency kidney care department was done at our setting, a tertiary care public sector teaching hospital. **Results:** Of 4,700 admissions, 3,700 case records were analyzed. The majority (> 50%) were dialysis patients with fluid overload (pulmonary edema), sepsis, and catheter-related infections. In dialysis CKD patients, the predominant cause of seeking emergency care was fluid overload (40%) and sepsis (21%), whereas in AKI it was sepsis (25%) and obstructive uropathy (11%). **Conclusion:** Half the admissions to emergency kidney care comprised dialysis patients. Important causes for seeking emergency care were pulmonary edema and sepsis. A high mortality rate was observed in those with high Sequential Organ Failure Assessment (SOFA) scores upon admission.
