## Executive GIST (TL;DR)
- Chronic kidney disease (CKD) is a growing global health problem that disproportionately affects **rural** and underserved populations, particularly in low- and middle-income countries (LMICs). - Key barriers to CKD care in rural areas include geographical distance, a shortage of nephrology specialists, and inadequate healthcare infrastructure. - CKD is projected to become the fifth leading cause of death by 2040, highlighting the urgency of alternative care models aligned with SDG 3. - The COVID-19 pandemic demonstrated that **telemedicine**, including **telenephrology**, can be effectively used to deliver care, but uptake among underserved groups remains limited. - Evidence from resource-constrained and remote settings shows telenephrology implementations have produced shorter waiting times, reduced costs, decreased health-related travel, higher patient satisfaction, and lower mortality in some populations. - Successful telenephrology programs in remote areas indicate the challenges to wider adoption are surmountable when appropriately addressed. - Scaling telenephrology requires government support, investment in digital infrastructure, measures to bridge the **digital divide**, and active community engagement. - Policy attention to access, workforce distribution, and health equity is essential for telenephrology to reduce kidney health disparities in rural settings. - The review frames telenephrology as a promising technological solution for kidney health inequities but notes that systemic and contextual enablers are necessary for sustainable impact.

Managing patients with CKD presents significant challenges, including limited access to healthcare. Key contributing factors include geographical barriers, a shortage of nephrology specialists, and insufficient healthcare infrastructure. As CKD is projected to become the fifth leading cause of death by 2040, it is crucial to develop innovative alternatives to the traditional care model to help bridge the gap in care in line with SDG 3. **Recent findings:** Lessons learned from the COVID-19 pandemic include the successful use of telemedicine-telenephrology for healthcare delivery. However, uptake among underserved populations remains challenging. However, a review of evidence suggests that such challenges are surmountable, as telenephrology has been practised in some resource-constrained remote areas. The results across these populations included shorter waiting times, lower costs, reduced health-related travel, higher overall satisfaction, and lower mortality rates. **Summary:** These findings portray telenephrology as a technological solution for kidney health inequities. However, government support, digital infrastructure development, bridging of the digital divide, and community engagement are essential. **Keywords:** access to care; health inequity and policy; nephrology workforce; telemedicine and digital technologies; underserved populations.
