## Executive GIST (TL;DR)
- A retrospective service evaluation of North East Ambulance Service (NEAS) records covered 1 January 2022 to 31 December 2024 and identified 351 suspected **opioid-associated out-of-hospital cardiac arrest** (OA-OHCA) cases. - Inclusion required documented cardiac resuscitation plus either a recorded drug overdose aetiology or documented **naloxone** administration; the dataset came from structured fields in the NEAS ePCR system. - The cohort’s mean age was 42.9 years; 69.2% were male; these arrests rose as a proportion of all resuscitation-attempted OHCAs from 4.7% in 2022 to 5.9% in 2024. - Interventions recorded included naloxone in 87.2% of incidents, adrenaline in 86.9%, amiodarone in 9.4%, advanced airway management in 88.3%, intraosseous access in 75.5%, and defibrillation in 16.5%. - Spatial and socioeconomic analysis showed clustering in deprived areas: 71.8% of cases were located in IMD deciles 1–3, indicating concentration in the most deprived communities. - The authors and data indicate a clinical pattern consistent with predominantly asphyxial arrest physiology: frequent airway, ventilation, and vascular access measures with relatively infrequent defibrillation. - The evaluation provides a descriptive baseline for NEAS service planning and is presented as the empirical foundation for a planned qualitative study of paramedic decision-making in OA-OHCA. - The study is a service evaluation using routinely collected data; patient-level data remain controlled by NEAS, and anonymised extracts may be available on request subject to governance approval. - Ethical approval was obtained from Northumbria University (Project No. 8566; Approval date 19/05/2025). Funding was via an NIHR pre-doctoral fellowship; the funder had no role in design or reporting. - The preprint status on medRxiv means the findings have not been peer reviewed and should not be used to guide clinical practice.

Opioid-associated out-of-hospital cardiac arrest (OA-OHCA) presents distinct clinical and operational challenges for ambulance services, particularly amid the emergence of potent synthetic opioids and uncertainty over the role of naloxone during cardiac arrest. This service evaluation sought to characterise the incidence, clinical features, and deprivation patterns of suspected OA-OHCA attended by North East Ambulance Service NHS Foundation Trust (NEAS), and to describe their geographic distribution. Methods: A retrospective service evaluation was conducted using structured and reported fields from the NEAS electronic patient care record (ePCR) system between 1 January 2022 and 31 December 2024. Inclusion required a documented cardiac resuscitation together with either a reported aetiology of drug overdose, or documented naloxone administration. Data were analysed descriptively. Results: 351 cases met the inclusion criteria. The mean age was 42.9 years; 69.2% were male. Cases represented a rising proportion of OHCA where resuscitation was attempted by NEAS across the period (4.7% in 2022 to 5.9% in 2024). Naloxone was administered in 87.2% of incidents, adrenaline in 86.9%, and amiodarone in 9.4%. Advanced airway management was recorded in 88.3% and intraosseous access in 75.5%; defibrillation was performed in 16.5%. Cases were concentrated in the most deprived communities, with Index of Multiple Deprivation (IMD) deciles 1-3 comprising 71.8% of the cohort. Conclusion: Suspected drug-related cardiac arrests in the NEAS region predominantly affected working-age adults in deprived communities, and were managed with consistent airway, ventilation, vascular access, and naloxone administration alongside infrequent defibrillation, consistent with a predominantly asphyxial arrest model. This evaluation provides a descriptive baseline that could inform service planning and provides the empirical foundation for a subsequent qualitative study of paramedic clinical decision-making in OA-OHCA. ### Competing Interest Statement The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper. The views expressed are those of the authors and not necessarily those of the NIHR or the Department of Health and Social Care. Employment: BK is a paramedic employed by North East Ambulance Service NHS Foundation Trust (NEAS), the ambulance service whose routinely collected records were evaluated. GM holds a joint appointment at Northumbria University and NEAS. JS declares no interests beyond his academic appointment at Northumbria University. Supervisory relationship: GM & JS are members of BK's NIHR PCAF supervisory teams. Data access: NEAS is the data controller for the source records and granted access for the purposes of service evaluation.
