## Executive GIST (TL;DR)
- Retrospective cohort of 373 neonates with birth asphyxia admitted to Jimma University Medical Center; data were extracted from medical records using a structured checklist. - Distribution by HIE stage: 121 (32.4%) Stage I, 169 (45.3%) Stage II, 83 (22.3%) Stage III. - Overall mortality reported as 9.1%; median event-free survival time 30.6 days. - Kaplan-Meier survival probabilities: 0.99 at day 1, 0.75 at day 20, and 0.29 at day 40 (95% CIs reported in source). - Survival differed across HIE stages (log-rank χ² = 45.4; p = 0.00) with worse survival for higher stages. - Cox proportional hazards identified predictors of mortality: Stage II HIE, acute kidney injury, Apgar score ≤ 3, Apgar score 4–6, outborn status, and neonatal sepsis. - Ordinal logistic regression identified factors associated with increasing HIE severity: outborn status, acute kidney injury, macrosomia, and hypoglycemia; maternal gestational diabetes showed a borderline association. - Investigators used Kaplan-Meier, log-rank, Cox regression, and ordinal logistic regression; statistical significance set at p < 0.05. - The cohort and analyses were from a single center in Southwest Ethiopia (JUMC); investigators recommend strengthening referral systems, improving perinatal care, and early detection and management of neonatal complications to improve outcomes.

Evidence regarding HIE severity, survival outcomes, and predictors of mortality remains limited in Ethiopia. This study assessed the severity, survival outcomes, and predictors of mortality among neonates with HIE admitted to Jimma University Medical Center (JUMC), Southwest Ethiopia. **Methods:** A retrospective cohort study was conducted among 373 neonates diagnosed with HIE following birth asphyxia and admitted to Jimma University Medical Center(JUMC). Data were extracted from medical records using a structured checklist. Kaplan-Meier survival analysis and the log-rank test were used to estimate and compare survival experiences. Cox proportional hazards regression identified predictors of mortality, while ordinal logistic regression was employed to determine factors associated with increasing HIE severity.
