Pathogen specific outcomes in this population remain poorly defined. The objective was to evaluate the epidemiology, viral etiology, and clinical severity of respiratory viral infections in hospitalized children with IEMs. Method: This single-center retrospective study included children with IMDs hospitalized with acute infectious illness and laboratory-confirmed respiratory viral detection during 2018-2024. Demographic features, underlying metabolic diagnoses, viral pathogens, and outcomes were evaluated. Result: A total of 45 infection episodes among 41 children were included; median age was 4 years (IQR:0.5-9.5), and 31 patients (75.6%) were male. The most common IEMs categories were amino acid metabolism disorders (36.6%) and complex molecule degradation and processing defects (26.8%). Respiratory disease was the predominant presentation (77.8%), followed by gastroenteritis (8.9%), sepsis (6.7%), and meningoencephalitis (6.7%). Acute metabolic decompensation occurred in 11/23 infection episodes in metabolically vulnerable patients (47.8%). Los niños con errores congénitos del metabolismo (ECM) tienen mayor riesgo de infecciones virales graves que pueden precipitar una descompensación metabólica aguda, el ingreso a la unidad de cuidados intensivos pediátricos y la muerte. La evolución específica según el patógeno en esta población no está bien definida. El objetivo fue evaluar la epidemiología, la etiología viral y la gravedad clínica de infecciones respiratorias virales en niños con ECM hospitalizados. Población y métodos. Este estudio unicéntrico retrospectivo incluyó niños con ECM hospitalizados con enfermedades respiratorias agudas y detección de virus respiratorios confirmada por laboratorio, durante 2018-2024. Se evaluaron datos demográficos, diagnóstico metabólico subyacente, patógeno viral y evolución clínica. Resultados. Se incluyeron 45 episodios infecciosos en 41 niños; la mediana de edad fue 4 años (RIC: 0,5-9,5 años) y 31 pacientes (75,6 %) fueron de sexo masculino. Las categorías más frecuentes de ECM fueron los trastornos del metabolismo de los aminoácidos (36,6 %) y los defectos del procesamiento de degradación de moléculas complejas (26,8 %). La enfermedad respiratoria fue la presentación predominante (77,8 %) seguida de gastroenteritis (8,9 %), sepsis (6,7 %) y meningoencefalitis (6,7 %). La descompensación metabólica aguda ocurrió en 11/23 (47,8 %) episodios infecciosos en pacientes metabólicamente vulnerables. El virus influenza fue el identificado con mayor frecuencia (35,6 %), seguido del rinovirus (22,2 %), el coronavirus estacional (CoVe) (13,3 %) y el virus sincicial respiratorio (11,1 %). El 13,3 % (6/45) de los casos requirieron cuidados intensivos pediátricos. La mortalidad relacionada con la infección fue del 6,7 % (n = 3) y en todos los casos había coinfección por CoVe. Conclusión. El patógeno viral identificado con más frecuencia fue influenza, mientras que CoVe se detectó en los 3 casos de muerte relacionada con la infección.
