## Executive GIST (TL;DR)
- The investigators performed a retrospective case-control analysis using samples from four prospectively collected Japanese cohorts; they compared 34 adults with **sepsis** to 46 adults with clinically similar sepsis mimics. - They measured 92 inflammation-related proteins with an **Olink** panel; the report states no protein reached significance after false discovery rate (FDR) correction, with the smallest FDR-adjusted P value = 0.345. - The primary prediction algorithm was an elastic-net model. Cross-validated, participant-averaged predictions produced an area under the ROC curve (AUC) of 0.442 (95% CI, 0.315–0.574), indicating performance around chance. - The study team also evaluated random forest and gradient boosting classifiers; these models similarly provided no clear discrimination beyond chance, and sensitivity analyses (all-serum and QC-pass subsets) yielded confidence intervals that included chance performance. - Authors concluded that a single measurement from an inflammation-focused protein panel displayed limited ability to distinguish sepsis from mimics; they noted integration with clinical data and serial sampling remains to be evaluated. - Ethical approvals were obtained for the parent cohorts and for this secondary analysis (Ethics Review Committee of Nagoya University Graduate School of Medicine, approval number 2023-0314). Participant-level data are not publicly available; aggregate protein-level results are in Supplementary Data. - Competing interests were declared by several authors, including industry collaborations and lecture/consulting fees; these relationships are listed in the source and described as outside the submitted work. - The authors recommend further work combining biomarkers with clinical variables and longitudinal measurements to determine whether discrimination can be improved.

We evaluated whether inflammatory protein profiles could distinguish these conditions in a retrospective case-control study using 4 prospective Japanese cohorts. The study included 34 adults with sepsis and 46 with sepsis mimics. We measured 92 proteins using Olink, assessed between-group differences with false discovery rate (FDR) adjustment, and evaluated 3 prediction models using repeated nested cross-validation. No protein met the FDR threshold; the lowest FDR-adjusted P value was 0.345. The primary elastic-net model had an area under the receiver operating characteristic curve of 0.442 (95% confidence interval, 0.315-0.574), based on cross-validation predictions averaged for each participant. Random forest and gradient boosting also showed no clear evidence of discrimination above chance. Confidence intervals included chance performance in both the all-serum and quality-control-Pass sensitivity analyses. In this cohort, a single measurement of an inflammation-focused protein panel showed limited discrimination between sepsis and sepsis mimics. Integration with clinical information and longitudinal measurements warrants further investigation. ### Competing Interest Statement D.K. reports collaborative research with Toray Research Center, Chugai Pharmaceutical Co., Ltd., and Otsuka Pharmaceutical Co., Ltd.; consulting fees from TXP Medical; and research grants from Asahi Kasei Medical, the YOKOYAMA Foundation for Clinical Pharmacology, the MSD Life Science Foundation, the Pfizer Health Research Foundation, the Hori Sciences & Arts Foundation, the Noguchi Institute, the General Insurance Association of Japan, the Aichi Kidney Foundation, and the Institutes of Innovation for Future Society, Nagoya University.
