## Executive GIST (TL;DR)
- The study compared **fine needle aspiration cytology** (FNAC) with **histopathology** using the Bethesda system as the FNAC classification framework. - Investigators reviewed 304 patient records from three Hawassa health facilities; each patient had FNAC and histopathology from the same site. - Data collection period spanned 30 October 2024 to 29 October 2025, and records were analyzed with cross-tabulation and χ² testing. - FNAC produced determinate/diagnostic results in 81.6% of the reviewed cases. - Reported sensitivity for FNAC was 88.2% (CI 77.6–95.1%); specificity was 94.4% (CI 90.6–97.1%). - False negative and false positive rates were 2.4% and 4.4%, respectively; positive predictive value 80.3% (CI 68.7–89.3%); negative predictive value 96.8% (CI 93.8–98.7%). - Overall diagnostic accuracy for discriminating benign from malignant thyroid lesions was 93.1%. - Authors stated FNAC shows strong agreement with histopathology and recommended improvements in sample handling, use of ultrasound guidance, and strict adherence to **Bethesda** criteria. - Ethical approval was obtained from Hawassa University IRB (reference IRB/372/16); the dataset is available from the institution and corresponding author. - The article is a medRxiv preprint and has not undergone peer review; authors cautioned against using the findings to guide clinical practice without further validation.

endocrine disorders worldwide, including in Ethiopia. They are characterized by alterations in hormone secretion, enlargement of the thyroid gland, or both. Fine needle aspiration cytology is an important initial screening tool, and histopathologic examination is essential for the definitive diagnosis of thyroid disease. This study aimed to compare the diagnostic accuracy of fine needle aspiration cytology with histopathology in identifying thyroid diseases. Methods: A cross-sectional study was conducted at three health facilities in Hawassa City, Ethiopia, involving 304 patients who underwent both thyroid fine needle aspiration cytology and histopathologic examination of the same site between 30th October 2024 and 29th October 2025. FNAC results were classified using the Bethesda system, with histopathology as the gold standard. Data were entered into Kobo Toolbox, exported to SPSS version 27, and analyzed using cross-tabulation and chi-square (χ²) tests to determine diagnostic accuracy. Result: 304 patient records were included in the study. The fine needle aspiration cytology was determinate/diagnostic in 81.6% of cases. The study revealed the sensitivity and specificity of fine needle aspiration cytology to be 88.2% (CI 77.6-95.1%) and 94.4% (CI 90.6-97.1%), respectively. The false negative and false positive rates in this study were 2.4% and 4.4%, respectively. The positive predictive value was 80.3% (CI 68.7-89.3%), and the negative predictive value was 96.8% (CI 93.8-98.7%). The accuracy of fine needle aspiration cytology in differentiating benign from malignant thyroid lesions was 93.1%. Conclusion: FNAC demonstrated strong agreement with histopathology and remains a reliable, cost-effective first-line diagnostic tool for thyroid disease, particularly in resource-limited settings. Strengthening sample handling, ultrasound guidance, and adherence to Bethesda criteria may further improve diagnostic precision ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: Ethical clearance and approval of the study was obtained from the institutional review board (IRB) of Hawassa University College of Medicine and Health Science with reference number IRB/372/16. A formal letter was obtained from HUCSH's chief clinical director and the Department of Pathology to get patients' medical record charts and collect report formats. The confidentiality of all patient records was maintained. This research involved only retrospective review of medical records without direct patient contact therefore, adherence to international norms such as the Declaration of Helsinki was not applicable. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable.
