## Executive GIST (TL;DR)
- The authors used longitudinal electronic health records to develop a counterfactual Structural Causal Model to estimate **individual treatment effects (ITE)** for **SGLT2 inhibitors** from observational data. - The model assumes counterfactual stability and implements a **Gumbel-Max** sampling mechanism to represent stochastic treatment assignment while keeping inference differentiable. - Optimal therapy choices were defined by maximizing the model-derived counterfactual value of future diagnostic outcomes rather than relying on observed prescribing patterns. - Evaluation used a doubly robust value estimator; investigators reported that the Counterfactual Gumbel-Max SCM outperformed real-world prescriptions in predicted treatment value across diagnostic categories. - The analysis identified patient subpopulations where **SGLT2 inhibitors** showed higher predicted counterfactual benefit, indicating heterogeneity of treatment effects in routine care. - This work is a retrospective, anonymized record analysis spanning routine-care data collected between 2000 and 2021; the source did not provide explicit sample sizes or detailed cohort demographics in the abstract. - Ethical approvals were obtained from TUKEB (approval number 44549-2/2018/EKU) and the University of Debrecen committees (approval number 6697-2024); informed consent was waived under applicable regulations. - Data are not publicly available because of ethical restrictions, but the corresponding author may provide access on reasonable request. - The report is a medRxiv preprint and has not been peer reviewed; the authors caution that findings should not be used to guide clinical practice.

Electronic health records now offer longitudinal data that can support data-driven therapy selection. We focus on SGLT2 inhibitors, medications with broad metabolic and cardiovascular benefits, whose clinical impact varies significantly based on individual patient profiles. We propose a counterfactual Structural Causal Model (SCM) to estimate individual treatment effects (ITE) of SGLT2 inhibitors from observational data. The model assumes counterfactual stability and uses a Gumbel-Max sampling mechanism to capture stochastic treatment assignment while enabling differentiable inference. Optimal treatments are defined by maximizing the counterfactual value of future diagnostic outcomes. We evaluate the Counterfactual Gumbel-Max SCM using a doubly robust value estimator and show that, across diagnostic categories, it outperforms real-world prescriptions in predicted treatment value, highlighting subpopulations where SGLT2 inhibitors yield superior outcomes. By integrating causal inference with electronic health data, this framework provides individualized counterfactual reasoning about SGLT2 treatment effects and can support clinicians in identifying optimal therapies for type 2 diabetes and comorbid conditions. ### Competing Interest Statement AN, DS, BD were employed by Black Horse Group Ltd. The remaining authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: This study did not involve any experimental intervention on human participants; it is a retrospective analysis of anonymized clinical records collected as a by-product of routine care between 2000 and 2021. The study was approved by the Scientific and Research Ethics Committee of the Hungarian Medical Research Council (TUKEB; approval number 44549-2/2018/EKU) and by the Regional and Institutional Research Ethics Committee of the University of Debrecen (DE RKEB/IKEB; approval number 6697-2024), as part of the University of Debrecen Clinical Center's data mining and machine learning initiative. All methods were carried out in accordance with the relevant guidelines and regulations and with local legislation and institutional requirements. Informed consent was waived by the above-named ethics committees, in accordance with national legislation and institutional requirements, because the study used anonymized data derived from routine clinical care. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable. Yes ## Data Availability The datasets generated and/or analysed during the current study are not publicly available due to the terms of the ethical approvals governing patient-derived hospital data (TUKEB 44549-2/2018/EKU; DE RKEB/IKEB 6697-2024), but are available from the corresponding author on reasonable request. ## Funder Information Declared Ministry of Culture and Innovation of Hungary from the National Research, EKOP KDP-24-1- BME-15, OTKA- K139330 Copyright The copyright holder for this preprint is the author/funder, who has granted medRxiv a license to display the preprint in perpetuity.
