## Executive GIST (TL;DR)
- A recent literature review in the Journal of the Endocrine Society examined possible links between **PMOS/PCOS** and **Alzheimer’s disease and dementia**, using a PubMed search through October 2025. - Investigators reported that people with PMOS demonstrate mild, reproducible deficits in executive function and memory on clinical testing, alongside observable brain structural and functional changes. - The review notes that any apparent association is likely mediated by metabolic comorbidities—chiefly insulin resistance and obesity—rather than an independent effect of PMOS itself. - Authors inferred that common PMOS-related conditions (diabetes, cardiovascular disease, stroke, obesity) are plausible drivers of later cognitive decline; the review did not present new primary data. - The review team included Ricardo Azziz, MD, MPH, MBA, Mina Amiri, Armita Kakavand Hamidi, Daniel L. Farkas and Richard P. Buyalos; funding came from the Foundation for Research and Education Excellence. - Prevalence context: PMOS affects an estimated 10%–15% of women worldwide, the review reminded, and has lifelong multisystem effects. - The literature search methods were summarized as comprehensive PubMed review to October 2025; the source did not report specific inclusion/exclusion criteria, effect sizes, or meta-analytic statistics in this news summary. - Investigators identified knowledge gaps and prioritized areas for future research to clarify mechanisms and disentangle direct PMOS effects from metabolic comorbidities. - Clinical implication suggested by authors: screening and managing cardiometabolic risk factors in PMOS patients may be the pragmatic route to reduce later dementia risk, though the review did not specify screening protocols. - The article emphasized the need for targeted studies to test causal pathways linking PMOS, metabolic dysfunction, and Alzheimer’s-related brain changes; specific trial designs were not described in the source.

October 13, 2026 Print this article Share this article ![](https://endocrinenews.endocrine.org/wp-content/uploads/jes_8_6cover-scaled-e1791207087954.jpeg) Alzheimer’s disease and dementia may be more common in women with polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome (PCOS), according to a review of existing evidence published in the _Journal of_ _the_ _Endocrine_ _Society._ PMOS affects 10% to 15% of women worldwide and is linked to a wide range of health issues, including diabetes, heart disease, stroke, depression, anxiety, infertility and endometrial cancer. The condition can have lifelong effects and may also affect males, but in different ways. “Clinically, PMOS patients exhibit mild but reproducible impairments in executive function and memory, alongside structural and functional brain alterations, which could suggest greater risk for Alzheimer’s disease and dementia. Current evidence suggests that any association between PMOS and Alzheimer’s disease and dementia is likely mediated by metabolic comorbidities, particularly insulin resistance and obesity, rather than an independent effect of PMOS itself,” said study co-author Ricardo Azziz, MD, MPH, MBA, of the University of Alabama at Birmingham in Birmingham, Ala. The researchers conducted a comprehensive PubMed search, from inception through October 2025 and inferred people with PMOS may be at greater risk for changes in memory, thinking, brain health, and pathways linked to Alzheimer’s disease. They also deduced that these risks may be related to common health conditions that often occur alongside PMOS, such as obesity and metabolic disorders. The review highlights important gaps in knowledge and identifies key areas for future research. “Understanding how these two disorders that significantly impact women’s health worldwide are inter-related presents the opportunity to not only understand what these two very important morbidities have in common but also to discover new pathways and causes which could guide the development of novel treatments for these devastating diseases,” Azziz said.
