## Executive GIST (TL;DR)
- Study evaluated whether risk of **sarcopenia** (measured by **SARC-F**) is associated with greater postprandial glycemic excursion (PPBS minus FBS) in older adults with **type 2 diabetes** and controlled fasting glucose. - Cross-sectional analysis applied to an outpatient dataset of 300 adults aged ≥50 years with FBS 90–130 mg/dL on stable oral therapy. - Primary exposure: SARC-F score and categorical high-risk vs low-risk groups. Primary outcome: glycemic excursion (BG = PPBS − FBS). - 64 patients (21.3%) had high-risk SARC-F scores. Mean age overall was 67.6 ± 6.8 years. - Mean BG (postprandial rise) was 119.4 ± 29.1 mg/dL in the high-risk group versus 74.8 ± 22.8 mg/dL in the low-risk group (difference 44.6 mg/dL; 95% CI 36.8–52.5; p < 0.0001). - SARC-F score and BG were strongly positively correlated (Pearson r = 0.777, p < 0.0001). - In multiple linear regression adjusted for age, diabetes duration, FBS and drug regimen, each 1-point rise in **SARC-F** score associated with a 10.2 mg/dL higher BG (95% CI 8.6–11.7). - The high-risk SARC-F group was ~6 years older; age and diabetes duration attenuated but did not eliminate the association. - Authors conclude the analysis plan can detect and quantify a link between sarcopenia risk and **postprandial** glucose rise and suggest treatment strategies should include measures to restore the mechanical glucose sink (eg, resistance training) and consider age-appropriate pharmacologic adjustments. - Study is a preprint (not peer reviewed); ethics approval was reported from the Ethics Committee of R.G. Kar Medical College Kolkata. Data available on request.

Muscle is the main site of glucose disposal after a meal. Fasting blood sugar (FBS) mostly reflects glucose output from the liver, whereas postprandial blood sugar (PPBS) depends more on insulin driven glucose uptake by muscle. Objective: To show how the association between sarcopenia risk and postprandial glycemic excursion would be tested in older adults with T2DM and controlled fasting glucose. Methods: A cross sectional design is planned for adults aged 50 years or more with T2DM, FBS between 90 and 130 mg/dL and a stable oral regimen. Sarcopenia risk is measured with the SARC F questionnaire. Glycemic excursion (BG) is PPBS minus FBS. The method was applied to a dataset of 300 patients collected over an OPD setting. Groups were compared with Welchs t test, the relationship between SARC F score BG was tested with Pearsons correlation, and multiple linear regression adjusted for age, diabetes duration, FBS and drug regimen. Results: In the patient dataset (mean age 67.6 +/ 6.8 years), 64 patients (21.3%) had a high risk SARC F score. BG was 119.4 +/ 29.1 mg/dL in the high risk group and 74.8 +/ 22.8 mg/dL in the low risk group (difference 44.6 mg/dL, 95% CI 36.8 to 52.5; p The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
