## Executive GIST (TL;DR)
- Retrospective cohort from the Wenzhou Glaucoma Progression Study examined 118 eyes (77 patients) with **normal-tension glaucoma** and at least 2 years of follow-up and five visual field tests. - Investigators used 3D OCT imaging to map optic nerve head (ONH) strains under three loading conditions: 20° **adduction**, 30° abduction, and acute intraocular pressure (IOP) elevation via 0.80 N force on the lower eyelid. - Effective strain measures were computed for the global ONH and segmented tissues (pre-lamina, lamina cribrosa, choroid, sclera) using a validated 3D strain-mapping algorithm. - Myopic NTG eyes showed higher ONH strains than non-myopic eyes during acute IOP elevation and abduction; reported means and P values were taken directly from the source. - In non-myopic eyes, strains differed significantly by loading condition, with adduction and acute IOP elevation producing larger strains than abduction. The myopic group showed less condition-dependent variation. - Multivariable linear mixed-effects models (adjusting for age, spherical equivalent, baseline perimetric MD) found that greater adduction-induced ONH strain in myopic NTG associated with faster MD decline (estimate reported in source). - Paradoxically, larger IOP-induced strains in myopic eyes correlated with slower progression in the same models (estimate and P value reported in source). - Tissue-level results: adduction-induced strains in the choroid and pre-lamina, and IOP-induced strains across ONH tissues, correlated with perimetric progression in myopic eyes. - Study limitations include retrospective design and preprint status; authors recommended further investigation into gaze-related optic nerve traction as a non-IOP biomechanical factor in myopic NTG.

Design: Retrospective clinical cohort study. Participants: A total of 118 NTG eyes from 77 patients with ≥ 2 years of follow-up and 5 VF tests were enrolled, including 66 eyes with myopia and 52 eyes without. Methods: Each participant underwent three-dimensional (3D) optical coherence tomography (OCT) imaging of the ONH under three mechanical loading conditions: 20° adduction, 30° abduction and acute intraocular pressure (IOP) elevation by applying a 0.80 N force to the lower eyelid. Effective strains across the global ONH complex and within specific segmented tissues (pre-lamina, lamina cribrosa, choroid, and sclera) were quantified using a validated 3D strain-mapping algorithm. Results: Myopic NTG eyes exhibited significantly higher ONH strains compared to non-myopic eyes under acute IOP elevation (10.1±2.7% vs. 9.3±1.8%, P = 0.044) and abduction (9.2±2.3% vs. 8.4±2.1%, P = 0.048). In the non-myopic group, ONH strains varied significantly across conditions (P The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
