## Executive GIST (TL;DR)
- The investigators analyzed European-ancestry participants from the Adolescent Brain Cognitive Development Study (ABCD; 3,523 children) and the All of Us Research Program (AoU; 302,421 adults) to study rare exonic variation. - They summed selection coefficients across genes separately for rare protein-truncating variants (PTVs) and for damaging missense variants, producing gene-constraint–weighted genetic loads (GLs) for each individual. - In ABCD, one additional constrained gene with a **PTV** corresponded to a 0.22 SD lower cognition score and was associated with **ASD** (odds ratio 1.80), investigators reported. - In AoU, the same per-gene **PTV** GL effect associated with **ASD** (OR 1.34), **schizophrenia** (OR 1.51) and **intellectual disability** (ID; OR 2.02), the study team observed. - Missense-derived GLs produced smaller but directionally concordant associations with cognition and diagnoses compared with PTV GLs. - Effect magnitude increased with gene constraint; the study reported nearly ninefold odds of ID for variants in the most constrained genes. - Carriers of a constrained **PTV** constituted 12% of AoU participants but accounted for 31% of ID cases, 20% of schizophrenia cases and 17% of **ASD** cases in that cohort. - Genetic loads were essentially uncorrelated with standard polygenic scores, and PTV GL retained 71–125% of its European-ancestry effect in African-ancestry participants compared with 23–62% for matched polygenic scores, the authors noted. - The authors declared employment and equity ties to Herasight, Inc., and reported institutional approvals and use of de-identified ABCD and All of Us controlled-tier data; study-level summary statistics are available on request subject to data-use rules.

In European-ancestry participants of the Adolescent Brain Cognitive Development Study (ABCD, 3,523 children) and the All of Us Research Program (AoU, 302,421 adults), we summed, separately for rare PTVs and damaging missense variants, the selection coefficients of affected genes into genetic loads (GLs). In ABCD, one additional typical constrained gene with a PTV was associated with 0.22 SD lower cognition and with ASD (odds ratio 1.80). In AoU, it was associated with ASD (1.34), schizophrenia (1.51) and intellectual disability (ID, 2.02). Missense GL showed smaller, concordant associations. Risk rose with gene constraint, to almost ninefold odds of ID in the most constrained genes, and carriers of a constrained PTV (12% of AoU participants) made up 31% of ID, 20% of schizophrenia and 17% of ASD cases. GL was essentially uncorrelated with polygenic scores. In African-ancestry participants, PTV GL retained 71-125% of its European-ancestry effect, versus 23-62% for matched polygenic scores. Constraint-weighted rare variant load contributes measurably to cognitive and neuropsychiatric outcomes in population cohorts. ### Competing Interest Statement M.A., S.M., J.S., J.A. and T.W. are employees of Herasight, Inc. and hold equity in the company.
