## Executive GIST (TL;DR)
- Investigators evaluated two commercial **decision-only AI** systems (Jev and OpenAI Decisions) and a large language model, **GPT-6 Luna**, across synthetic clinical vignettes and public benchmark sets. - The evaluation comprised 2,134 case presentations and 2,483,222 total model calls, according to the preprint authors. - The team tested how adding nonclinical text (labels or pressure cues) changed model outputs without altering clinical findings; these manipulations produced measurable framing sensitivity. - In the controlled challenges, label/pressure cues altered decisions by 13.4 more changes per 100 for the decision models and 14.0 more changes per 100 for the LLM versus neutral wording. - Authors paired each decision model with the LLM in a two-step workflow and observed reduced framing sensitivity: paired performance showed 2.6 additional changes per 100 in development. - Pairing reduced sensitivity by about 81% in development; held-out reports showed reductions in the 42%–48% range, as reported by the study team. - Triage accuracy for the workflows remained near 81%–82% across conditions in the preprint’s experiments. - The paired Decisions workflow had low operational cost in the authors’ calculations: at most US$0.77 per 1,000 entries. - Data, prompts, comparison mappings and analysis code were made publicly available in the project repository; the study used only synthetic or previously published benchmark data. - Authors noted the work as an early proof of concept to reduce framing sensitivity and identify influential text for human review; the preprint cautions that this is not peer-reviewed and should not guide clinical practice.

We tested the clinical consistency and potential bias susceptibility of the two available decision models, Jev and OpenAI Decisions, against a large language model (LLM), GPT-6 Luna, then paired each decision model with the LLM. Across 2,134 case presentations and 2,483,222 model calls, we measured decision changes after adding labels or pressure cues without changing clinical findings. In the original controlled challenges, these additions changed 13.4 more decisions per 100 than neutral wording for the decision models and 14.0 for the LLM. Pairing reduced this to 2.6, about 81% lower in development and 42% to 48% lower in held-out reports. Triage accuracy stayed near 81% to 82%. The paired Decisions workflow cost at most US$0.77 per 1,000 entries. We see this as a practical proof of concept for reducing framing sensitivity and flagging influential text for review. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
