## Executive GIST (TL;DR)
- Investigators used a benefit-transfer approach to estimate avoided **healthcare expenditure** tied to residential **greenspace** across Washington state. - They applied decile-specific relative healthcare-cost ratios drawn from a Northern California Kaiser Permanente cohort of about five million members. - The study mapped those ratios onto population-weighted distributions of satellite-derived **normalized difference vegetation index** (NDVI) across Washington census block groups. - Ratios were calibrated to Washington’s 2020 statewide per-capita healthcare expenditure so estimates reflect the state’s own cost baseline. - Estimated annual per-capita costs fell from $6,472.75 in the least-green decile to $5,939.39 in the greenest decile, a per-person difference of $533.35. - Simulated interventions produced modeled statewide annual savings: $45.6 million if the lowest decile rose to the next decile; $157.7 million if the four lowest deciles were lifted to Decile five; and $56.9 million for greening urban cores in the five largest Washington cities. - Authors noted these are associations modeled via transferability assumptions; the analyses depend on NDVI as an imperfect proxy for meaningful nature access. - The preprint used aggregated public data only and did not require IRB oversight; data and code will be made available on request or deposited after acceptance. - Key uncertainties include transferability between regions and the lack of causal inference from the applied benefit-transfer framework.

To contribute to these investigations, we estimated potential avoided healthcare expenditures associated with residential greenness in Washington using a benefit-transfer framework. We applied decile-specific relative healthcare-cost ratios from a Northern California cohort of approximately five million Kaiser Permanente members to population-weighted distributions of satellite-derived normalized difference vegetation index across Washington census block groups. Ratios were calibrated to Washington’s 2020 statewide per-capita healthcare expenditure, allowing estimates to reflect the state’s own cost baseline. Estimated annual per-capita costs declined from $6,472.75 in the least-green decile to $5,939.39 in the greenest, a difference of $533.35. Simulated interventions yielded estimated annual statewide savings of $45.6 million from raising the lowest decile to the next decile, $157.7 million from lifting the four lowest deciles to Decile five, and $56.9 million from urban-core greening in Washington’s five largest cities. These estimates are modeled associations, not causal forecasts, and depend on transferability between regional populations and on normalized difference vegetation index as an incomplete proxy for meaningful nature access. Nonetheless, the findings highlight increased and equitable greening efforts as a potentially consequential component of preventive health and infrastructure policy. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: This study used only publicly available, aggregated data (satellite-derived vegetation indices, U.S. Census block-group population counts, and published statewide healthcare expenditure) together with previously published rate ratios from a peer-reviewed study. It did not involve human participants, identifiable personal information, or contact with individuals. No institutional review board or other oversight body approval was therefore sought or required. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable.
