## Executive GIST (TL;DR)
- The article is a practice pointer aimed at healthcare professionals, authored by a multinational clinical and public health team, focusing on clinician actions before, during, and after **extreme weather** events. - Investigators reported a five-fold increase in weather-related disasters over the past 50 years and stated global surface temperature is heading over 1.5°C, increasing hazard frequency and unpredictability. - Clinicians should assess patients’ exposure to weather hazards and identify vulnerability factors that raise risk of adverse outcomes. - The study team observed that clinicians and patients should co-develop an emergency plan and kit, review essential drugs and devices, and adjust regimens for anticipated hazards. - During disasters, data indicated priorities: maintain continuity of care where safe, monitor hazard-specific warning systems, protect staff and family safety, and triage high acuity cases. - Authors noted ensuring access to essential treatments — specifically **insulin**, **dialysis**, and **oxygen** — is a priority during acute events. - After an event, clinicians should confirm physical and psychological safety of staff and infrastructure before returning to normal operations, and ask patients about missed doses, missed appointments, and mental health impacts. - The article includes a clinical vignette: rural subarctic clinician during an extreme **wildfire** season managing a 32-year-old with childhood **asthma**, vital signs and exam findings are reported in the source. - The authors emphasise that hazards are becoming more frequent and are likely to be rapid and hard to predict; clinicians must plan despite uncertainty. - The full text and infographic were referenced by the article for operational details; the short source did not report granular protocols, dosing changes, or stepwise operational checklists.

Ask patients about missed doses of drugs, missed appointments, and mental health impacts _You are a family doctor working in a clinic in a remote part of the Canadian subarctic region during the most severe wildfire season on record. The nearest safe town is a seven hour drive away. The wildfire plan had always been to shelter in place, however temperature and precipitation patterns have changed, and the fire is approaching quickly. Evacuation rumours are circulating._ _A 32 year old patient with a history of childhood asthma arrives complaining of shortness of breath and cough which worsens during thick smoke. On examination, his temperature is 36.9 °C, heart rate 80/min, respiratory rate 20/min, blood pressure 130/80 mm of Hg, and O2 saturation 94%. He has scattered expiratory wheezes, but no signs of respiratory distress._ Weather related disasters have increased five-fold over the past 50 years, driven by climate change.1 The average rise in global surface temperature is heading over 1.5°C, which could result in further rapid and difficult to predict hazards.23 Healthcare providers are increasingly called on to anticipate and respond to extreme weather events such as hurricanes, typhoons, drought, wildfires, … [View Full Text](https://www.bmj.com/content/395/bmj-2026-100635.full) ## Log in ### Log in using your username and password [BMA Member Log In](https://bmjverification.bma.org.uk/validate/userdetails?url=https%3A//www.bmj.com/content/395/bmj-2026-100635%3Frss%3D1%26variant%3Dshort%26sso%3D "Log in here if you are a BMA member.")
