## Executive GIST (TL;DR)
- The authors applied a staggered 2x2 **Difference-in-Differences** design to estimate causal effects of multiple Ghana health policies on annual mortality from 1990–2023. - Policies evaluated include **CHPS** (1999), the **National Health Insurance Scheme**, **Free Maternal Care** (2008), **GEHIP** (2012), and the **National Newborn Health Strategy (NNHS)** (2019). - Two comparator sets were used: the global average and the lower-middle-income country group; event-study plots assessed the parallel trends assumption. - For **child mortality**, investigators reported a 9.5% reduction after **GEHIP** relative to the global trend, and a 16.4% reduction associated with **NNHS**, though the NNHS window displayed mild pre-period drift so that estimate is an upper bound. - For **maternal mortality**, the most credible estimate tied to **GEHIP** showed a 7.2% reduction below the global comparator, supported by a near-flat pre-period ATT series in event-study checks. - The **Free Maternal Care** policy produced no detectable improvement in maternal mortality versus either comparator; authors note this aligns with prior evidence that access reforms need sufficient facility capacity to lower mortality. - **CHPS** produced a large apparent maternal mortality effect, but investigators invalidated it due to a strong pre-existing downward trend in the event-study window. - Authors used publicly available aggregated data from the World Bank World Development Indicators; no individual-level data or IRB approval was required. - The preprint has not been peer reviewed; authors declared no competing interests and provided data source links in the manuscript.

Prior evaluations have relied on descriptive trend analysis and cross-sectional comparisons, without constructing a credible counterfactual. This paper applies a staggered 2x2 Difference-in-Difference approach to estimate the causal effect of each policy on annual mortality outcomes over the period 1990 to 2023, comparing Ghana’s trajectory against the global average and the lower-middle-income country group as comparators. Parallel trends are assessed through event-study plots of annual treatment effects. For child mortality, GEHIP (2012) reduced mortality by an estimated 9.5 percent relative to the global trend, and NNHS (2019) by 16.4 percent, though mild pre-existing drift in the NNHS window means the latter is an upper bound. For maternal mortality, GEHIP again produces the most credible estimate at 7.2 percent below the global comparator, supported by a near-flat pre-period ATT series. The Free Maternal Care policy (2008) produced no detectable improvement in maternal mortality relative to either comparator, consistent with evidence that access reforms require adequate facility capacity to translate increased attendance into reduced mortality. CHPS (1999) generates a large apparent maternal mortality effect that is invalidated by a strong pre-existing downward trend in the event-study window. Taken together, the results indicate that supply-side and quality-focused interventions have driven Ghana’s most credible mortality improvements, while financial access reforms have had their strongest effects on child rather than maternal survival. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Not Applicable The details of the IRB/oversight body that provided approval or exemption for the research described are given below: This study used only publicly available, aggregated secondary data from the World Bank World Development Indicators database.
