## Executive GIST (TL;DR)
- Purpose: The study evaluated whether initiation of **GLP-1 receptor agonists (GLP-1RAs)** is associated with changes in anti-VEGF treatment burden among patients with **diabetic macular edema (DME)** using the American Academy of Ophthalmology **IRIS Registry**. - Design: Retrospective cohort study using IRIS Registry data. Incident GLP-1RA initiators were matched 1:1 to non-initiators via Mahalanobis distance matching to balance sociodemographics, DME risk factors, and comorbidities that influence GLP-1RA prescribing. - Cohort size: 9,896 matched pairs (N = 19,792 total patients). - Time window: Monthly anti-VEGF injection frequency was assessed across a 36-month window: 12 months before through 24 months after GLP-1RA initiation. - Analysis: A longitudinal mixed-effects event-study model evaluated monthly injection trajectories, with adjustment for DME duration. Visual acuity (VA) and central subfield thickness (CST) were prespecified secondary outcomes. - Primary outcome: No statistically significant difference in anti-VEGF injection trajectories between GLP-1RA initiators and matched controls after initiation (interaction coefficients ranged from -0.18 to 1.59; P > 0.05). - Secondary outcomes: No significant differences in VA (range -0.05 to 0.04 logMAR; P > 0.05) or CST (range -14.12 to 33.58 µm; P > 0.05) between cohorts. - Interpretation: In these matched IRIS Registry cohorts, initiating a GLP-1RA was not associated with reduced anti-VEGF injection burden or measurable improvements in VA or CST over the assessed follow-up. - Data source & ethics: Analyses used de-identified, HIPAA-compliant IRIS Registry data. The Partners Human Research IRB determined the project did not meet the definition of human subjects research. The study was funded by the Massachusetts Eye and Ear Clinical Data Science Fund. - Publication status: Preprint on medRxiv; not peer-reviewed. Details such as specific GLP-1RA agents, dosing, adherence, and other clinical management variables were not reported in the source abstract.

Methods: Incident GLP-1RA initiators were matched 1:1 with controls via Mahalanobis distance matching (9,896 pairs; N=19,792) on sociodemographics, DME risk factors, and factors influencing GLP-1RA prescription including hypertension, obesity, chronic kidney disease. A longitudinal mixed-effects event-study model evaluated monthly anti-VEGF injection frequency over a 36-month window (12 months before through 24 months after initiation), adjusting for DME duration. Visual acuity (VA) and central subfield thickness (CST) were secondary outcomes. Results: Following GLP-1RA initiation, anti-VEGF injection trajectories did not significantly differ between the matched GLP-1RA and control cohorts (interaction coefficients -0.18 to 1.59, P>0.05). Likewise, no differences in VA were observed between cohorts (-0.05 to 0.04 logMAR, P>0.05) or CST (-14.12 to 33.58 µm, P>0.05). Conclusion: In these matched cohorts, GLP-1RA initiation was not associated with the trajectory of anti-VEGF use or changes in VA or CST. Precis We used the American Academy of Ophthalmology IRIS® Registry (Intelligent Research in Sight) to identify patients with DME. In 19,792 matched patients, there was no significant reduction in injection frequency post GLP1-RA initiation and no significant change in VA or CST. ### Competing Interest Statement R.P.S. has received consulting fees from AbbVie, Alcon, Apellis, Astellas (Iveric Bio), Genentech/Roche, Regeneron, Topcon, ANI Pharmaceuticals and research support from Janssen ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
