## Executive GIST (TL;DR)
- STAT investigation finds that **health care costs** are shifting financial strain onto taxpayers and public employees through rigid state rules and political decisions. - The story highlights Dauphin County, Pennsylvania, where officials raised property taxes by 22% in December 2024 to help cover climbing employee health care expenses. - Over two decades in Dauphin County, the workforce was reduced by 19% while county health care spending more than doubled and **per-employee** costs tripled. - County leaders faced a choice between raising taxes or cutting employee health benefits for a shrinking staff who rely on those benefits. - The piece frames the trend as widespread across the country, driven by state policies and political dynamics that limit local options for containing costs or restructuring benefits. - Reporting methods included interviews with more than a dozen state and local government leaders, health policy experts, and industry executives, and review of government meetings, contracts, health plan documents, and administrative complaints and appeals. - The article is part of STAT’s “Out of Pocket, Out of Reach” series; portions of the reporting beyond the lead examples are available only to STAT+ subscribers and are not included in the publicly available excerpt. - Author Bob Herman covers health insurance and hospitals and conducted the reporting for this piece; contact and subscription details are provided in the source.

For the first time in two decades, the county’s leaders had not only voted to raise their property taxes, but did so by a whopping 22%. What they may not have realized: That higher tax bill was needed to help offset the soaring health care costs of the county’s employees. Advertisement Over the past 20 years, even as Dauphin County cut its workforce by 19%, its health care spending more than doubled. Per-employee costs tripled. If the county didn’t raise property taxes, officials would have had to consider slashing health benefits for a dwindling staff who rely on them.
