## Executive GIST (TL;DR)
- Five large employers — **Amazon**, Axon Enterprise, Google, Meta and Oracle — joined a government-backed statement to implement **electronic prior authorization** in their self-funded health plans. - The CMS and Department of Labor announced the pledge, with employers committing to push carriers and third-party administrators to enable digital prior authorizations immediately. - The firms will align with the pledge’s target that electronic processes be available by the start of next year; health plans have set a Jan. 1 deadline for FHIR-based implementation. - The effort relies on HL7 Fast Healthcare Interoperability Resources (**FHIR**) APIs to allow real-time checks of prior authorization requirements and documentation. - CMS is already moving toward FHIR API requirements for Medicare Advantage, Medicaid/CHIP MCOs and insurers on the federally facilitated exchanges, per the source. - Participating health plans pledged to answer at least 80% of electronic prior authorization requests in real time next year through these APIs. - Self-funded plans are governed by ERISA and were largely excluded from some prior federal reform efforts; the pledge aims to bring employers into the modernization effort. - Over half of Americans are covered by employer self-funded plans; the source notes **Amazon**’s plan covers 2.8 million workers and is the largest single-employer health plan. - Investigators reported that some insurers have already reduced prior authorizations by 11% according to plan statements; nevertheless, some providers say they have not seen practical improvements. - The announcement extends prior voluntary pledges from nearly 60 health plans to eliminate some prior authorizations, clarify denials, and respond in real time.

The cost-controlling method used by insurers requires providers to get a preapproval for certain services and prescriptions before administering them to patients. Insurers say prior authorization is a necessary cost control, while providers say it delays needed care. Insurers and regulators have made progress with reforming prior authorizations in Medicare, Medicare Advantage, Medicaid and commercial plans. However, self-funded health plans have been left out of many efforts, including the pledge, because they aren’t subject to federal health mandates, being governed instead by the Employee Retirement Income Security Act. The latest additions to the pledge will expand ongoing efforts at the federal level to improve prior authorization — a priority for both the CMS and DOL, which works directly with companies to offer health benefits to their employees. Over half of Americans receive coverage through employers with self-funded health plans. Amazon alone covers 2.8 million workers, making its self-funded plan the [largest single-employer health plan in the country](https://hrexecutive.com/amazon-now-operates-the-largest-singleemployer-health-plan-in-the-us/). “We’ve brought insurers, providers, and technology companies to the table to modernize prior authorization, and now employers are helping us take another important step,” CMS Administrator Dr. Mehmet Oz said in a press release. “When the entire healthcare system moves together, we can spend less time on paperwork and more time getting patients the care they need.” The companies will piggyback on the pledge’s goal of implementing electronic prior authorizations by the start of the year, with the statement calling for the insurance carriers and third-party administrators that oversee self-funded plans to make digital processes available to them immediately. Health plans have set their own Jan. 1 deadline to switch to a prior authorization process that relies on HL7’s Fast Healthcare Interoperability Resources, or FHIR, a data exchange standard for healthcare information. FHIR is open source, meaning it can be universally used by modern health technology systems for real-time data sharing. This effort aligns with an upcoming [CMS requirement that will require FHIR-based application programming interfaces for prior authorizations](https://www.healthcaredive.com/news/cms-final-prior-authorization-rule-payer-deadline/704721/) among Medicare Advantage plans, Medicaid and Children’s Health Insurance Program managed care organizations, and insurers on federally facilitated exchanges. These insurers will need to have APIs that enable providers to instantly see whether a service requires prior approval and what documentation is required by Jan. 1. Health plans also pledged to have at least 80% of electronic prior authorization requests answered in real time next year, primarily through these FHIR APIs. Those plans say they’re [reduced prior authorizations by 11% so far](https://www.healthcaredive.com/news/insurer-prior-authorization-commitment-update-ahip-bcbsa/816819/). Still, some providers say [they have yet to feel any improvement from the pledges](https://www.techtarget.com/revcyclemanagement/feature/What-providers-really-need-to-streamline-prior-authorizations). ### Recommended Reading * [ Health insurers, nudged by Trump administration, pledge reform to prior authorization ](https://www.healthcaredive.com/news/health-insurers-pledge-prior-authorization-reform-trump-hhs/751309/) By [Rebecca Pifer Parduhn](https://www.healthcaredive.com/editors/rparduhn/) • June 23, 2025 * [ CMS finalizes rule tightening prior authorization turnaround for insurers ](https://www.healthcaredive.com/news/cms-final-prior-authorization-rule-payer-deadline/704721/) By [Rebecca Pifer Parduhn](https://www.healthcaredive.com/editors/rparduhn/) • Jan. 17, 2024 * [ ![](https://www.healthcaredive.com/static/img/social_icons/article_page/bookmark-add.svg?111915221026) Add us on Google ](https://www.google.com/preferences/source?q=https://www.healthcaredive.com) * [ ![](https://www.healthcaredive.com/static/img/social_icons/share-icon.svg?111915221026) Share ](javascript:void\(0\)) * [ ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/share-link-box.svg?111915221026) Copy link ](javascript:void\(0\)) * ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/email.svg?111915221026) Email * [ ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/linkedin.svg?111915221026) LinkedIn ](https://www.linkedin.com/shareArticle?mini=true&url=https%3A//www.healthcaredive.com/news/five-major-companies-pledge-faster-electronic-prior-authorizations/833848/&title=Five%20major%20companies%20pledge%20faster%2C%20electronic%20prior%20authorizations) * [ ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/x.svg?111915221026) X/Twitter ](https://twitter.com/intent/tweet?text=Five%20major%20companies%20pledge%20faster%2C%20electronic%20prior%20authorizations%20https%3A//www.healthcaredive.com/news/five-major-companies-pledge-faster-electronic-prior-authorizations/833848/%20via%20@healthcaredive) * [ ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/facebook.svg?111915221026) Facebook ](https://www.facebook.com/sharer/sharer.php?u=https%3A//www.healthcaredive.com/news/five-major-companies-pledge-faster-electronic-prior-authorizations/833848/) * [ ![](https://www.healthcaredive.com/static/img/social_icons/share_dropdown/print.svg?111915221026) Print ](javascript:window.print\(\)) * [ ![](https://www.healthcaredive.com/static/img/social_icons/article_page/cart.svg?111915221026) License ](https://www.healthcaredive.com/selfservice/article-licensing/submit/?newspostUrl=https://www.healthcaredive.com/news/five-major-companies-pledge-faster-electronic-prior-authorizations/833848/) Filed Under: [Payer](https://www.healthcaredive.com/topic/payers/), [Government](https://www.healthcaredive.com/topic/healthcare-regulation/) ### Healthcare Dive news delivered to your inbox Get the free daily newsletter read by industry experts Email: * Select Newsletter: Daily Dive M-F * Select Newsletter: Payer Weekly Every Wednesday * Select Newsletter: IT Weekly Every Thursday * Select user consent: By signing up to receive our newsletter, you agree to our [Terms of Use](https://www.informatechtarget.com/terms-of-use/) and [Privacy Policy](https://www.informatechtarget.com/privacy-policy/).
