## Executive GIST (TL;DR)
- This sub-analysis of a randomized controlled trial examined the effect of intermittent one Hertz **neuromuscular electrical stimulation (NMES)** of the **common peroneal nerve** in patients after forefoot surgery. The analysis included 42 patients: 26 received NMES plus standard of care (SOC) and 16 received SOC alone. - Primary postoperative assessments at 14 days included wound healing (complete vs not complete), edema measured by the **figure-of-eight (FO8)** method, and patient-reported outcomes using the Manchester-Oxford Foot Questionnaire (MOXFQ). - At 14 days, complete wound healing occurred in 77% of patients receiving NMES plus SOC versus 40% in the SOC-only group; this difference reached statistical significance (p<0.05). - Edema reduction was greater in the NMES group, described as a 74% relative reduction compared with SOC; this result was statistically significant (p=0.02). - The trial intervention used intermittent one Hz stimulation; the device evaluated was manufactured by Firstkind Ltd (the authors disclosed employment or consultancy links to the company and declared competing interests). - The report notes MOXFQ was used to assess patient-reported outcomes, but the abstract does not present MOXFQ results in this sub-analysis. - The sub-analysis is derived from ClinicalTrials.gov registration NCT04927234 and is a preprint (not peer reviewed); authors caution it should not guide clinical practice until peer review. - Ethics approvals were obtained from multiple bodies (Hospital Clinic de Barcelona; Hospital Universitari Vall d'Hebron; West of Scotland REC 4; Western Institutional Review Board/WIRB). Data are available from the corresponding author on reasonable request. - Funder information is declared: Firstkind Ltd. The authors state adherence to ethical guidelines and trial registration requirements.

A recent randomized controlled trial (ClinicalTrials.gov [NCT04927234](https://www.medrxiv.org/lookup/external-ref?link_type=CLINTRIALGOV&access_num=NCT04927234&atom=%2Fmedrxiv%2Fearly%2F2026%2F08%2F24%2F2026.08.21.26361007.atom)) demonstrated improved outcomes with intermittent one Hertz (Hz) neuromuscular electrical stimulation (NMES) of the common peroneal nerve. This sub-analysis evaluates its effect in patients undergoing forefoot surgery. Forty-two patients undergoing forefoot procedures were included; 26 received NMES plus standard of care (SOC) and 16 received SOC alone. Wound healing was assessed at 14 days. Edema was measured using the figure-of-eight (FO8) method. Patient-reported outcomes were assessed using the Manchester-Oxford Foot Questionnaire (MOXFQ). At 14 days, complete wound healing occurred in 77% of patients receiving NMES plus SOC compared with 40% in the SOC group (p The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
