## Executive GIST (TL;DR)
- The **Paisa Kids** Study is building a longitudinal cohort of 3,000 youth (ages 6–15) from the Paisa region of Colombia to study risk and resilience pathways to **Serious Mental Illness (SMI)**, defined as schizophrenia, bipolar disorder, and severe depression. - Recruitment enriches transdiagnostic risk: high-risk youth are those with a first- or second-degree adult relative with SMI or youth with EMR-identified psychiatric diagnoses linked to later SMI; lower-risk youth have relatives without SMI. - Baseline phenotyping includes psychiatric and developmental history, neurobehavioral testing, assessment of environmental exposures and adverse childhood experiences (ACEs), and **genome-wide genotyping**; longitudinal follow-up uses psychiatric electronic medical records (EMRs). - As of the report, 2,384 participants were enrolled (mean age 11.2 years, 45.0% female); 2,211 were recruited as high SMI-risk and 173 as lower SMI-risk. - Overall, 73.2% of enrolled youth met criteria for a history of one or more psychiatric disorders at baseline. - Comparative analyses found higher rates of depressive and anxiety disorders, attention-deficit/hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), and several ACEs among high-risk versus lower-risk youth. - The study integrates multidomain phenotyping with genomic data and longitudinal EMR linkage to enable investigation of transdiagnostic versus disorder-specific developmental pathways to SMI. - The protocol received ethical approvals from the University of Washington and HOMO hospital, complies with Colombian regulations and the Declaration of Helsinki, and data will be accessible through the NIH Data Archive. - This report is a medRxiv preprint (not peer reviewed) posted October 7, 2026; funder includes the National Institute of Mental Health (R01MH127165).

Its objective is to clarify the early clinical, neurobehavioral, genetic, and environmental profiles associated with later SMI and distinguish transdiagnostic from disorder-specific pathways. This paper describes the study infrastructure, recruitment streams, and baseline assessment battery, and characterizes the first two-thirds of the cohort. Method: Youth aged 6-15 years are being recruited. Youth recruited for high SMI risk either have a first or second degree adult relative with a SMI diagnosis or a psychiatric diagnosis themselves identified through electronic medical records (EMRs) associated with later SMI. Lower-risk youth have a relative without a SMI diagnosis. Baseline measures include psychiatric and developmental history, neurobehavioral functioning, environmental exposures, and genome-wide genotyping. Psychiatric EMRs provide longitudinal follow-up. Results: Among 2,384 participants enrolled thus far (mean age=11.2 years, 45.0% female), 2,211 were recruited for high SMI-risk and 173 for lower SMI-risk. Overall, 73.2% met criteria for a history of one or more psychiatric disorders. Logistic regression comparing rates of diagnoses and ACEs between recruitment streams showed elevated rates of depressive and anxiety disorders, attention-deficit/hyperactivity disorder, oppositional defiant disorder, and several ACEs among high versus lower SMI-risk youth. Conclusion: Integrating multidomain phenotyping, genomic data, and longitudinal EMR follow-up, the Paisa Kids cohort provides a unique resource for investigating pathways of risk and resilience to SMI. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: The study complied with regulations for human subjects research, as mandated by Colombian laws and the Declaration of Helsinki, and was approved by the research ethics committees at the University of Washington and HOMO hospital. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable.
