The 11 health boards on the mainland will be merged into two bodies covering the west and east of the country under the proposals. The three existing health boards for Shetland, Orkney, and the Western Isles could be merged with local councils, but details have yet to be confirmed. Final decisions will be taken only after consultation. Swinney set out a brief outline of his plans for “simplifying” the health service in Scotland in his programme for government speech on 1 September.[1](https://www.bmj.com/content/394/bmj-2026-100751.short?rss=1#ref-1) “This will be the most significant structural reform of the NHS in the era of devolution,” Swinney told the Scottish parliament. “Alongside targeted investment and our national flow plan, it will enable us to deliver the additional capacity to ensure that by the end of 2031 no one waits more than 26 weeks for the treatment that they need.” The leader of the Scottish National Party said that the Scottish government “will undertake radical and widespread public service reform.” “We will require all public services to make a decisive shift to prevention and early intervention,” he said. He also called on the public sector in Scotland to use technology more effectively. “Staff working on the frontline should be encouraged, and enabled, to break through silos between organisations in order to respond to the real life needs and experiences of the people that they serve,” he said. But reaction from doctors’ groups was muted. Iain Kennedy, chair of British Medical Association Scotland, said, “Disappointingly, given the scale of the changes proposed, we have very little detail on these reforms beyond the broad headlines provided in the programme for government.” He said that the union will await further information so it can provide full and thorough scrutiny of the proposals. “Evidence of how these changes will improve care for patients and not simply be about saving money must be central to what does ultimately come forward,” he said “The voice of doctors and NHS staff must be heard and taken into account in what should be a full, open, and honest period of consultation, which gives the public and local communities across Scotland the chance to have their say. “Local accountability and the flexibility to make local decisions that respond to issues like demography and geography must be preserved in any potential plans.” Lilian Macer, the Scottish secretary for the public sector union, was more critical, “You cannot just restructure your way out of a public service crisis.” She added: “The Scottish government must build the capacity of public services, not spend millions reorganising them. “Previous overhauls have shown this rarely delivers the long term savings promised. They’re loaded with upfront costs, cause years of disruption, and are a distraction from making desperately needed improvements to our services.” “Changes to NHS boundaries won’t lead to a single extra nurse, care worker, or paramedic nor [to] any new care packages or hospital beds. “Merging public services and reducing NHS boards will take decisions further away from the patients, staff, and communities who best understand local needs. It must only ever happen if there’s a clear, lasting benefit to services. The main long term savings are likely to be made through employing fewer people. But Swinney told BBC’s Radio Scotland Breakfast that there would be no compulsory redundancies and he would “work collaboratively” with trade unions on a reform programme. ## References 1.
