## Executive GIST (TL;DR)
- This retrospective analysis used the Premier PINC AI Healthcare Database to compare a minimally manipulated human umbilical tissue allograft (**STRAVIX**) with four acellular cellular and acellular matrix products (**aCAMPs**) in hospitalized patients with **diabetic lower extremity ulcers (DLEU)**. - The study cohort included 2,626 DLEU patients treated with STRAVIX and 10,002 patients treated with aCAMPs; each patient’s earliest inpatient encounter was the index admission. - To adjust for baseline differences, inverse probability of treatment weighting (**IPTW**) was applied and IPTW-adjusted generalized linear models estimated clinical and economic outcomes. - STRAVIX-treated patients had significantly lower odds of post-discharge **infection reaction**, **wound dehiscence**, and both **minor and major amputations** compared with aCAMP-treated patients (specific effect sizes not reported in the source abstract). - Follow-up resource use was reduced in the STRAVIX group: DLEU-related follow-up visits were 17–23% lower, debridement-related visits were 19–23% lower, and **all-cause readmission** incidence was 22–25% lower versus aCAMP patients. - Hospital utilization showed modest improvements with STRAVIX: length of stay was 3% lower and cost of all-cause visits was 20–25% lower compared to aCAMP-treated patients. - The authors report the dataset as large and conclude improved clinical outcomes when STRAVIX was used in addition to standard surgical management compared to certain acellular CAMPs. - The report is a medRxiv preprint and has not undergone peer review; authors J. Banerjee, C.-C. Chen, and T. Styche are employees of Smith & Nephew, which is declared as a competing interest. - Ethical declarations state that deidentified data were HIPAA-compliant and the study was exempt from IRB approval; additional trial registration or reporting checklist details were asserted as followed but specific identifiers were not reported in the abstract.

The objective of this study was to use real-world evidence to compare the clinical and economic outcomes of hospital care for DLEU patients treated with a minimally manipulated human umbilical tissue (STRAVIX◊) or other acellular CAMPs. Methods: This retrospective analysis was conducted on patients with DLEU treated with the CAMPs using data from the Premier PINC AI Healthcare Database. Each patient's earliest inpatient encounter was designated as the index admission. The analysis groups included a cellular human umbilical tissue allograft (STRAVIX) and four different acellular CAMPs (aCAMPs). To balance baseline covariates, inverse probability of treatment weighting (IPTW) was applied. Subsequently, IPTW-adjusted generalized linear models were utilized to estimate healthcare-related outcomes and hospital-related costs. Results: 2,626 DLEUs patients with STRAVIX and 10,002 DLEUs patients with aCAMPs were identified. Compared to the patients treated with aCAMPs, the patients who had STRAVIX in addition to standard of care had significantly lower odds of having an infection reaction, dehiscence, minor and major amputations, after index admission discharge. The number of DLEU related follow-up visits was 17-23% lower, debridement related visits were 19-23% lower and the incidence of all-cause readmission was 22-25% lower in Stravix patients compared to aCAMP patients. The length of hospital stay days was 3% lower and the cost of all-cause visits was 20-25% lower in STRAVIX treated patients compared to aCAMP patients. Conclusion: This large data set demonstrates improved clinical outcomes in patients who had STRAVIX in addition to standard surgical management, when compared to certain acellular CAMPs. ### Competing Interest Statement JB, CCC and TS are employees of Smith & Nephew ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
