## Executive GIST (TL;DR)
- This randomized, double-blind, placebo-controlled multicenter superiority trial evaluated **Anas barbariae 200K (Oscillococcinum®)** versus placebo for acute influenza-like illness (ILI) in primary care in India in 2021. - Participants were children (6–12 years) and adults (18–65 years) presenting with ILI symptoms of less than 24 hours’ duration and were randomized to treatment given three times daily for three days. - The primary endpoint was the proportion of participants with alleviation of ILI symptoms within 72 hours after treatment initiation. - 707 participants were randomized and treated: 352 received **Anas barbariae 200K** and 355 received placebo. - Within 72 hours, 74% of the **Anas barbariae 200K** group had symptom alleviation versus 44.5% in the placebo group (p < 0.0001), meeting the primary endpoint. - Secondary outcomes favored the active treatment: faster mean time to symptom alleviation (46.7 vs 67.3 hours; p < 0.0001) and shorter median time to return to daily activities by Kaplan–Meier analysis (4.0 vs 6.0 days; p < 0.0001). - No serious adverse events were reported; the study reports no safety signal identified. - Trial registration: Clinical Trials Registry of India CTRI/2021/02/031250 (registered February 12, 2021). - Funding and competing interests: study declared funder Boiron (France); several investigators received fees for enrolling/treating participants and some authors were employed by Laboratoires Boiron. - Ethical approvals were obtained from multiple independent ethics committees; data are available on reasonable request to the corresponding author.

This study aimed to evaluate its efficacy in alleviating ILI symptoms within 72 hours. Secondary outcomes assessed symptoms relief, adverse effects, functional outcomes, compliance, and participant and investigator satisfaction. Methods This prospective, randomized, double-blind, placebo-controlled multicenter superiority trial with two parallel groups was conducted in primary care in India in 2021. Children (6–12 years) and adults (18–65 years) with ILI symptoms for less than 24 hours were randomized to receive either _Anas barbariae_ 200K (Oscillococcinum®) or placebo. Treatment was given three times daily for three days. The primary endpoint was the proportion of participants with alleviation of ILI symptoms within 72 hours following the initiation of treatment. Results A total of 707 participants received _Anas barbariae_ 200K (n = 352) or placebo (n = 355).
