## Executive GIST (TL;DR)
- Retrospective cohort from five New York City hospitals using EHR query (2022–2024) examined adult ED visits placed on a **sepsis** care pathway. - Investigators identified associations between demographics, vital signs, basic labs, ED diagnoses, and four favorable outcomes: short hospital length of stay (HLOS 36°C, absence of hypotension, WBC 4,000–15,000/µL, and lactate <2.0 mmol/L associated with multiple favorable outcomes; WBC 4,000–15,000/µL was the only factor linked to all four outcomes. - Age 18–49 had the strongest association with HLOS <72 hours and survival to discharge; lack of a severe ED diagnosis predicted no ICU stay; non-sepsis or non-infection ED diagnoses predicted no sepsis adjudication. - Fewer than one-quarter of pathway activations carried an ED diagnosis of **sepsis**. - Authors conclude certain clinical features may help identify lower-risk patients eligible for sepsis care de-escalation but note that predictive factors differ by specific outcome and that prospective validation is required.

Methods We conducted a retrospective cohort study using an electronic health record (EHR) data query from five hospitals in New York City (2022-2024). Multivariable logistic regression models were used to evaluate association of patient demographics, clinical characteristics, and ED diagnoses with four favorable reference outcomes: shorter hospital length of stay (HLOS), no intensive care unit (ICU) stay, survival to discharge, and no sepsis adjudication according to institutional quality-of-care standards. Results Among 9,443 ED visits, 45.9% of participants were 36C, absence of hypotension, WBC count 4,000-15,000/uL, and lactate The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
