## Executive GIST (TL;DR)
- Analytical treatment interruptions (**ATIs**) are required in many HIV cure–related trials, yet the impact of ATIs on participants' **health-related quality of life (HRQOL)** is not well characterized. - This study pooled data from two UCSF-affiliated HIV cure–related trials (UCSF SCOPE-ATI Study and UCSF-amfAR Combination Trial) to evaluate HRQOL trajectories during ATIs. - Seventeen people with HIV (PWH) were included; HRQOL was measured at baseline, pre-ATI, medication restart, and study completion. - Baseline psychosocial measures included validated scales of **depressive symptoms**, trait-based **anxiety**, and **resilience**. - Analyses used both frequentist and Bayesian linear mixed-effects models to assess change over time, associations with baseline psychosocial factors, and moderation by resilience. - Frequentist results showed a significant overall increase in HRQOL over time (β = 0.63, p = .016). - Higher baseline depressive symptoms (β = -1.00, p = .048) and higher anxiety (β = -1.18, p = .001) were each associated with lower HRQOL across time points. - **Resilience** moderated HRQOL change: the interaction time*resilience was positive (β = 0.36, p = .040), indicating participants with higher baseline resilience experienced steeper HRQOL improvements. - Authors conclude HRQOL improved over time despite ATI participation, and they emphasize the importance of identifying and addressing baseline psychosocial risks—particularly depression and anxiety—to support participant well-being in cure-related trials. - Keywords and indexing include Analytical treatment interruptions; Human Immunodeficiency Virus; cure research; psychosocial health; quality of life. - Study identifiers reported: PMID 42559869; DOI 10.1080/25787489.2026.2715800.

Our analysis included data from two HIV cure-related studies involving ATIs: the UCSF SCOPE-ATI Study and the UCSF-amfAR Combination Trial. HRQOL was measured at baseline, pre-ATI, medication restart, and study completion. Baseline depressive symptoms, trait-based anxiety, and resilience were assessed using validated scales. Frequentist and Bayesian Linear mixed-effects models evaluated changes in HRQOL, the influence of baseline psychosocial factors, and potential moderating effects. **Results:** In frequentist models, HRQOL significantly improved over time (_β_ = 0.63, _p_ = .016). Baseline depressive symptoms (_β_ = -1.00, _p_ = .048) and anxiety (_β_ = -1.18, _p_ = .001) were negatively associated with HRQOL, indicating that higher levels of these factors were linked to lower HRQOL. Resilience moderated improvements in HRQOL over time (time*resilience _β_ = 0.36, _p_ = .040), with higher baseline resilience associated with steeper increases in HRQOL. **Conclusions:** HRQOL improved over time in this sample despite ATI participation requirements. Findings underscore the need to address psychosocial factors, particularly depressive symptoms and anxiety, in HIV cure-related studies to optimize participant well-being and enhance the overall study experience. **Keywords:** Analytical treatment interruptions; Human Immunodeficiency Virus; cure research; psychosocial health; quality of life.
