## Executive GIST (TL;DR)
- Researchers conducted an ecological study examining **COVID-19 mortality** and **SARS-CoV-2 infections** across Austrian municipalities from January 2020 to June 2023. - The study employed zero-inflated negative binomial regression and random-effects meta-analysis to assess associations with demographic, socioeconomic, and environmental variables. - Among 18,430 reported COVID-19 deaths, mortality correlated positively with a higher proportion of elderly residents aged 85+, and negatively with home ownership rates. - Commuter proportions, older adult age groups (75-84 and ≥85), education levels, and employment status showed significant univariable associations with COVID-19 mortality. - Environmental factors such as precipitation and average temperature periods also displayed associations in univariable analyses but were less prominent in multivariable models. - Analysis of over 5.9 million documented infections found links to employment rates among 15–64 year olds, part-time employment, and nursing care allowance recipiency. - Variables like population aged 85+, income, education level, air pollution, and tourism metrics revealed inconsistent effects on infection rates across years. - The associations between demographic and socioeconomic factors and COVID-19 outcomes were relatively stable for mortality but more variable for infections over time. - Study data were drawn from national population records and required agency approval for access, maintaining patient confidentiality. - Ethical approval was granted by the Medical University of Graz ethics committee, and authors declared no conflicts of interest. - Findings emphasize persistent structural vulnerabilities influencing epidemic impact at the regional level in Austria.

It is unclear whether factors associated with regional COVID-19 mortality and SARS-CoV-2 infections changed over the course of the pandemic. Methods: We evaluated the association of ecological factors with municipality-specific COVID-19 mortality in the national population of Austria between January 2020 and June 2023. Analyses were conducted separately per calendar year and pooled across years via random-effects meta-analysis. Outcomes were analysed using univariable and multivariable zero-inflated negative binomial (ZINB) regression, including annually averaged environmental exposome exposure (including air pollution and UV radiation), socioeconomic, demographic and infection dissemination predictors with significance threshold set to p The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
