## Executive GIST (TL;DR)
- In June 2024 a cluster of **Mycobacterium abscessus** infections was reported at a hospital in Maryland, USA. The organism is frequently drug-resistant and causes severe disease in vulnerable patients in healthcare settings. - An investigation was conducted by hospital infection prevention staff together with the Maryland Department of Health, the Centers for Disease Control and Prevention, and the Wadsworth Laboratory of the New York State Department of Health. - Investigators used epidemiologic data, environmental sampling, and **whole-genome sequencing** to compare patient and environmental isolates. - Genomic analysis showed a strong association between patient and environmental isolates, with **0–24 single-nucleotide polymorphism (SNP) differences**, indicating likely patient exposure to the hospital environment as the source. - Mitigation measures initiated during the investigation included **tap water use restrictions**, installation of point-of-use water filters, hospitalwide water flushing, and staff education on **sink safety**. - The report states that initiation of these mitigation efforts likely prevented additional patient infections, although it notes that not all strategies are equally effective or sustainable. - Keywords and focus areas identified include antimicrobial resistance, biofilms, environmental exposure, water microbiology, molecular epidemiology, and infection control in hospitals. - The investigation and report are indexed (PMID: 42660585; DOI: 10.3201/eid3209.260093) and published in Emerging Infectious Diseases (Sept 2026, Vol. 32:1397–1401). - The findings underscore the role of hospital water systems as reservoirs for nontuberculous mycobacteria and the importance of combined epidemiologic and genomic approaches in outbreak investigations. - The source notes both the effectiveness of the interventions in this instance and the broader challenge of implementing sustainable mitigation approaches for hospital water-associated nontuberculous mycobacterial transmission.

In June 2024, a cluster of M. abscessus infection cases was reported at a hospital in Maryland, USA. An investigation involving hospital infection prevention staff, the Maryland Department of Health, the Centers for Disease Control and Prevention, and the Wadsworth Laboratory of the New York State Department of Health included initiation of mitigation efforts such as tap water use restrictions, point-of-use water filters, hospitalwide water flushing, and staff education regarding sink safety. Whole-genome sequencing of patient and environmental isolates, coupled with epidemiologic data, revealed a strong association (0-24 single-nucleotide polymorphism differences) between environmental and patient samples, indicating that infections were likely a result of patient exposure to M. abscessus. Ensuring initiation of mitigation initiatives likely prevented additional patient infections. Although effective in this investigation, not all mitigation strategies are equal or sustainable. **Keywords:** Maryland; Mycobacterium abscessus; United States; antimicrobial resistance; bacteria; biofilms; disease outbreaks; environmental exposure; hospitals; infection control; molecular epidemiology; nontuberculous mycobacteria; opportunistic infections; tuberculosis and other mycobacteria; water microbiology; whole genome sequencing.
