## Executive GIST (TL;DR)
- Trauma triggers an immediate, multidimensional activation of **innate and adaptive immunity** together with neuroendocrine, endovascular and metabolic systems, disrupting normal structure–function relationships. - The Review frames the immune response to injury as disturbances in three core biological hallmarks: barrier systems (delimitation), molecular communication, and physiological adaptation. - Patient-specific, injury-specific and modern care–specific determinants shape whether immune trajectories become adaptive (restitution) or maladaptive (chronic dysfunction or death). - Clinical trajectories after severe injury range from recovery to persistent immune dysfunction; contemporary rescue and intensive care strategies influence these outcomes. - Compartmentalization of inflammatory responses is emphasized: systemic measurements may not reflect localized immune states; this concept matters for interpretation and treatment. - Danger-associated molecular patterns (**DAMPs**) and other circulating mediators correlate with immune suppression in trauma patients, illustrating molecular links between tissue damage and altered immunity. - Multi-omics profiling of human trauma cohorts enables classification of patient **endotypes**, offering a mechanistic framework to guide spatiotemporal, precision **immunomodulation**. - The Review focuses exclusively on human studies to synthesize clinically relevant themes in trauma immunology and to guide future research directions. - Figures in the article illustrate determinants of the biological response, immunopathophysiological hallmarks, the immune response after severe trauma, and the effects of clinical interventions on these hallmarks. - The article situates current understanding within prior frameworks (danger theory, compartmentalization, consensus immune dysregulation) and summarizes key references linking trauma biology to clinical outcomes.

The immune response emerges from disturbances in overarching hallmarks of any living being—barrier systems, molecular communication and adaptation—further shaped by patient-specific, injury-specific and modern care-specific determinants. These factors drive adaptive or maladaptive immune trajectories, from restitution to chronic dysfunction or death. Multi-omics analyses of trauma cohorts now enable endotype discovery, providing a framework for mechanism-guided, spatiotemporal precision immunomodulation. This Review focuses exclusively on human studies to conceptualize major themes in clinical trauma immunology to guide future research.
