- This population-based cohort study analysed bacteraemia isolates from the Calgary Health Zone (CHZ) between 2006–2022, integrating clinical, antimicrobial prescribing, and whole-genome sequencing data for five major pathogens: **Staphylococcus aureus**, **Enterococcus faecalis**, **Enterococcus faecium**, **Escherichia coli**, and **Klebsiella pneumoniae**. - The dataset included 7,271 S. aureus, 1,609 E. faecalis, 801 E. faecium, 11,363 E. coli, and 2,319 K. pneumoniae isolates linked to bacteraemia episodes. - Genomic clusters (“strains”) were defined with StrainGST and assigned to sequence types (STs) or clonal complexes (CCs); strain-level incidence was modelled and stratified by community-onset (≤48 h) versus hospital-onset (>48 h). - Antimicrobial resistance (AMR) phenotypes were analysed alongside community and hospital antibiotic prescribing rates; temporal trends used average annual percentage change (AAPC) with negative-binomial and binomial regression. - Between 2010–2022 fluoroquinolone prescribing declined in the community (AAPC = -6.8%) and hospitals (AAPC = -5.1%), which coincided with marked reductions in fluoroquinolone resistance among Gram-positive species, notably **S. aureus**. - Clinically important resistance in S. aureus (cloxacillin, ciprofloxacin, erythromycin, clindamycin) declined overall from 2006–2022, driven mainly by decreases in hospital-onset cases (AAPC = -16.0%). - In E. coli, resistance to ceftriaxone and ciprofloxacin clustered in ST131 and the emerging ST1193; ST1193 incidence rose, particularly in community-onset infections (AAPC = 17.7%). - CTX-M-27-producing E. coli ST131 increased between 2008–2022 (AAPC = 23.8%), while CTX-M-14-producing ST131 declined between 2013–2022 (AAPC = -15.9%). - Community cephalosporin prescribing increased between 2010–2022 (AAPC = 7.3%), paralleling some gram-negative resistance and strain shifts. - For K. pneumoniae, hypervirulent ST23 was the most frequent lineage (N = 88) with an increasing incidence trend observed between 2006–2019 (AAPC = 3.0%, not statistically definitive given wide CI). - Conclusions emphasise that AMR dynamics vary by species and strain; stewardship must target multiple drug classes, genomic surveillance to detect emergent resistant strains, and community-targeted interventions beyond hospitals.

We aimed to assess how antimicrobial resistance (AMR) responds to prescribing changes and determine which bacterial strains shape these dynamics. Methods We analysed antibiotic prescribing rates, clinical and genomic data from 7,271 Staphylococcus aureus, 1,609 Enterococcus faecalis, 801 Enterococcus faecium, 11,363 Escherichia coli, and 2,319 Klebsiella pneumoniae isolates, associated with bacteraemia episodes in the CHZ between 2006–2022.
