## Executive GIST (TL;DR)
- Anti-glomerular basement membrane (**anti-GBM**) glomerulonephritis is an uncommon but severe cause of rapidly progressive glomerulonephritis in children, driven by autoantibodies against the glomerular basement membrane. - The reported case involves a 14-year-old girl presenting with acute-onset edema and hypertension; initial work-up demonstrated **hematuria**, **proteinuria**, hypoalbuminemia, and progressive decline in renal function, with serum anti-GBM antibodies positive. - Kidney biopsy confirmed diffuse crescentic glomerulonephritis with linear IgG deposition along the glomerular basement membrane, consistent with anti-GBM disease. - Treatment comprised high-dose corticosteroids, **plasmapheresis**, and **mycophenolate mofetil (MMF)** rather than a cyclophosphamide-based regimen. - After three months of therapy, clinical signs and laboratory markers improved and there was partial recovery of renal function. - The case underscores the importance of prompt recognition and rapid initiation of therapy in pediatric anti-GBM disease to limit irreversible renal injury. - The authors propose that an **MMF-containing combination regimen** may be a feasible alternative to cyclophosphamide in selected pediatric patients, though this is reported as a single case and broader data were not provided in the source. - Ethical and consent statements: institutional ethical oversight and guardian consent were obtained; authors declared no conflicts of interest.

The disease is characterized by the formation of antibodies against the glomerular basement membrane, leading to diffuse glomerular injury and rapid deterioration of renal function if not diagnosed and treated promptly. We report the case of a 14 year-old girl who was admitted with acute-onset edema and hypertension. Initial investigations revealed hematuria, proteinuria, hypoalbuminemia, progressive renal impairment, and positive anti-GBM antibodies. Kidney biopsy confirmed the diagnosis, demonstrating diffuse crescentic glomerulonephritis with linear IgG deposition along the glomerular basement membrane. The patient was treated with high-dose corticosteroids, plasmapheresis, and mycophenolate mofetil. After three months of treatment, both clinical and laboratory parameters improved, with partial recovery of renal function. This case highlights the importance of early recognition and prompt treatment of pediatric anti-GBM disease and suggests that an MMF-containing combination regimen may be a feasible alternative to cyclophosphamide-based therapy in selected patients. **Keywords:** Anti-glomerular basement membrane glomerulonephritis; Children; Kidney biopsy; Plasmapheresis; Rapidly progressive glomerulonephritis. © 2026.
