## Executive GIST (TL;DR)
- Investigators combined non-mitogenic **CD3 antibody** therapy with HLA‑A2–targeted **CAR‑Treg** treatment to address resistance from alloreactive memory T cells in transplantation settings. - The study team deleted the T‑cell receptor alpha chain (TRAC) in CAR‑Tregs and compared TRAC‑/‑ and TRAC+/+ CAR‑T cells under CD3 antibody challenge. - Data indicated TRAC deletion did not impair antigen‑specific activation, expansion, transcriptomic profile, phenotype, epigenetic marks, or functional readouts of CAR‑Tregs; the source reported these assessments but did not detail specific assays or doses. - TRAC‑/‑ CAR‑Tregs showed robust protection from CD3 antibody–mediated depletion in circulation and peripheral tissues, while most TRAC+/+ CAR‑T cells were eliminated. - The subset of TRAC+/+ CAR‑T cells that escaped depletion exhibited a transcriptomic signature consistent with **T‑cell exhaustion**, according to authors. - Authors noted that CD3 antibody therapy provided an in vivo survival advantage to TRAC‑/‑ CAR‑Tregs, shifting the immune balance toward regulation. - The abstract stated this combined strategy could reduce the CAR‑Treg cell dose required for clinical efficacy, potentially improving translational feasibility; specific clinical endpoints, dosing regimens, and safety data were not reported in the source. - The preprint authors declared inventorship on a related patent application; the work has not been peer reviewed, as indicated by the source.

An emerging approach using Chimeric Antigen Receptor Regulatory T cells (CAR-Tregs) targeting donor antigens, allows the suppression of alloimmune responses while preserving protective immunity. However, transplant recipients harbor a substantial pool of alloreactive memory T-cells, limiting CAR-Tregs tolerance-promoting effect. To address this challenge, our strategy combines selective depletion of activated T-cells, using non-mitogenic CD3 antibody therapy, with HLA-A2-targeted-CAR-Treg therapy. We established that T-cell receptor alpha chain (TRAC) deletion does not impair CAR-Treg antigen-specific activation, expansion, or their transcriptomic, phenotypic, epigenetic, and functional profiles. TRAC-/-CAR-Tregs exhibit robust protection from CD3 antibody depletion in circulation and peripheral tissues, whereas most TRAC+/+CAR-T cells are eliminated. Importantly, TRAC+/+CAR-T cells that escape CD3 antibody mediated depletion exhibit a transcriptomic signature consistent with T-cell exhaustion. Our findings demonstrate that CD3 antibody therapy provides an in vivo survival advantage to TRAC-/-CAR-Tregs, thereby shifting the immune balance toward regulation. This approach might enhance the translational potential of CAR-Treg therapy by reducing the required cell dose needed for clinical efficacy. ### Competing Interest Statement J.Z, T.B and S.C are listed as inventors on a patent application (EP22300582) related to the work presented in this manuscript.
