## Executive GIST (TL;DR)
- Investigators used a retrospective cohort design to emulate target trials across the TriNetX Linked Research Network, drawing electronic health record and claims data from 44 US health care organizations. - Eligible adults were **ACEI/ARB**-naive, aged 55–80 years, initiating an **ARB** or **lisinopril**; the source did not report additional enrollment criteria beyond age and prior ACEI/ARB exposure. - Separate 1:1 propensity-score matching was performed for each pairwise comparison; propensity variables and exact matching covariates were not detailed in the source. - Primary outcome: incident all-cause **dementia**. Secondary outcomes: Alzheimer disease, vascular dementia, cerebral infarction, death, and combined dementia or death. - Cox proportional-hazards models estimated hazard ratios (HRs) with 95% CIs; the source did not provide model covariate lists or proportionality testing results. - From 22,025,599 individuals screened, 2,051,275 met eligibility; matched cohort sizes ranged from 10,113 to 857,773. Median follow-up ranged 4.7–7.2 years across comparisons. - Pooled ARBs versus lisinopril: HR 0.86 (95% CI, 0.85–0.87) for incident all-cause dementia, indicating lower risk with pooled ARBs. - Individual ARBs showed heterogeneity; HRs across agents ranged from 0.66 to 0.93 versus lisinopril, indicating substantial agent-level variation in dementia association. - Within-class (non-losartan vs losartan) comparisons reproduced agent ordering by HRs and showed HR range 0.68–0.94; **losartan** had the least favorable association among commonly prescribed ARBs. - Secondary outcomes also exhibited agent-level variation; the source reported this but did not provide agent-specific numeric results for each secondary outcome in the abstract. - Sensitivity analyses included lagged and treatment-persistence analyses, negative-control outcomes, and E-values; authors reported robustness but did not list numerical sensitivity outputs in the abstract. - Authors concluded there is clinically relevant within-class heterogeneity in dementia associations across ARBs and noted confirmation in randomized trials is needed. - Data availability: individual-level TriNetX data are not publicly available; aggregate results can be requested from the corresponding author. - Ethical oversight: Weill Cornell Medicine determined IRB review was not required due to de-identified secondary data use; no patient consent was required.

Whether dementia risk differs across individual ARBs remains uncertain and was investigated in this study. METHODS: This retrospective cohort study emulated target trials using electronic health record and claims data from 44 US health care organizations in the TriNetX Linked Research Network. ACEI/ARB-naive adults aged 55 to 80 years initiating an ARB or lisinopril were eligible. Separate 1:1 propensity-score matching was performed for each comparison. Individual and pooled ARBs were compared with lisinopril (between-class), and non-losartan ARBs with losartan (within-class). The primary outcome was incident all-cause dementia; secondary outcomes included Alzheimer disease, vascular dementia, cerebral infarction, death, and dementia or death. Cox models estimated hazard ratios (HRs) and 95% CIs. Sensitivity analyses included lagged and treatment-persistence analyses, control outcomes, and E-values. RESULTS: Of 22,025,599 individuals screened, 2,051,275 were eligible; matched cohorts ranged from 10,113 to 857,773, with median follow-up of 4.7 to 7.2 years. Pooled ARBs were associated with lower dementia risk than lisinopril (HR, 0.86; 95% CI, 0.85 to 0.87), with substantial variation across individual agents (HR range, 0.66 to 0.93). Within-class comparisons showed similar heterogeneity (HR range, 0.68 to 0.94) and reproduced between-class agent ordering by HR estimates. Considerable agent-level variation was also observed across secondary outcomes. Losartan, the most commonly prescribed ARB, had the least favorable dementia associations. Negative-control associations were near null within class, and sensitivity analyses supported robustness. DISCUSSION: Individual ARBs were associated with differing dementia risks, suggesting clinically relevant within-class heterogeneity pending confirmation in randomized trials. ### Competing Interest Statement Dr. Ndhlovu serves as a scientific advisor to ViiV Healthcare, serves on the board of CytoDyn, and has financial interests in Ledidi AS, all for work unrelated to this study. Yes The details of the IRB/oversight body that provided approval or exemption for the research described are given below: Weill Cornell Medicine confirmed that institutional review board review was not required for this study because it involved secondary analysis of de-identified TriNetX data; informed consent was therefore not required. I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable. Yes ## Data Availability Individual-level data from the TriNetX Linked Research Network are not publicly available and cannot be shared by the authors.
