## Executive GIST (TL;DR)
- Prospective MRI study comparing 20 patients with **mucopolysaccharidosis** (17 MPS I, 3 MPS VI; mean age 19±7 years) to 19 age-matched healthy controls (20±7 years). - Investigators measured CSC macrostructure (cross-sectional area, diameters) and microstructure (fractional anisotropy [FA], radial diffusivity [RD], mean and axial diffusivities) using structural and diffusion MRI. - The patient cohort showed reduced CSC cross-sectional area and anterior–posterior diameter at upper cervical levels compared with controls; these were reported with statistical significance after FDR correction. - Diffusion metrics indicated widespread microstructural changes across the cervical spinal cord: lower FA and higher RD throughout the CSC in patients versus controls (pFDR<0.05). - Restricted foramen magnum space correlated with microstructural damage at lower cervical levels even when overt macrostructural compression was not present on conventional measures. - Morphological and diffusion metrics tracked severity of compression and clinical myelopathy (pFDR<0.05), according to the authors. - Lower cervical spinal cord integrity correlated with more advanced growth retardation in the patient cohort (reported p<0.001). - Among patients treated with hematopoietic stem cell transplantation, earlier treatment timing associated with preservation of upper CSC morphology (reported p<0.05). - The study team observed distinct neuroimaging phenotypes across MPS genotypes (pFDR<0.05). - Data and analysis code are available on request; code repository: https://github.com/umn-milab/mps-sc-resolve.

Early detection is critical to prevent irreversible neurological deficits and preserve quality of life. This prospective study aimed to identify quantitative neuroimaging biomarkers of structural CSC damage and myelopathy in MPS I and VI. Methods: Structural and diffusion magnetic resonance imaging (MRI) of the CSC was analyzed in 17 MPS I patients, 3 MPS VI patients (19±7 years), and 19 case-matched healthy controls (20±7 years). CSC macrostructure (cross-sectional area, diameters) and microstructure (fractional anisotropy [FA], radial [RD], mean, axial diffusivities) were evaluated within and beyond the maximal compression areas. Results: MPS patients demonstrated reduced CSC cross-sectional area and anterior-posterior diameter at upper cervical levels, alongside widespread microstructural alterations (lower FA, higher RD) throughout the CSC versus controls (pFDR ## Funder Information Declared National Institute of Neurological Disorders and Stroke, https://ror.org/01s5ya894, R03NS139000, U54NS065768, P30NS076408 University of Pennsylvania, https://ror.org/00b30xv10, MDBR-17-123-MPS National Institute of Biomedical Imaging and Bioengineering, P41EB027061 National Institutes of Health, 1S10OD017974-01 Copyright The copyright holder for this preprint is the author/funder, who has granted medRxiv a license to display the preprint in perpetuity.
