## Executive GIST (TL;DR)
- Study retrospectively reviewed consecutive mechanical thrombectomy cases at two high-volume stroke centers from January 2019 to December 2025, assessing the phenomenon of **passive advancement** of the aspiration catheter on stored fluoroscopy and angiography. - Dataset: 1,824 individual passes in 995 patients encompassing 1,027 occlusions; 733 passes had imaging adequate for assessment of passive advancement. - **Passive advancement** occurred in 525 of 733 assessable passes (71.6%); in 306 of 733 passes (41.7%) the catheter passively advanced to an arterial branch point. - On a per-pass basis, passes with passive advancement had substantially higher rates of successful reperfusion: eTICI ≥2b-3 occurred in 71.0% with passive advancement versus 19.7% without; eTICI ≥2c-3 occurred in 57.5% versus 10.1%. - When the catheter passively advanced to an arterial branch, success rates were higher still: eTICI ≥2b-3 was 90.5% with branch advancement versus 32.1% without; eTICI ≥2c-3 was 79.4% versus 18.7%. - The source text reports statistical comparisons (p values) but the provided excerpt is truncated and the exact p values are not fully reported in the source material supplied here. - Authors conclude that passive advancement was independently associated with superior recanalization outcomes and suggest that passive advancement should be a technical objective during aspiration thrombectomy. - Study limitations in the provided text: retrospective review and that some details (complete p values, full multivariable model coefficients, and other numerical results) are not available in the supplied excerpt. - Competing interests were declared for several authors, including consulting and equity relationships with multiple device and software companies; several authors reported no competing interests. - Data are available on reasonable request from the corresponding author, subject to IRB approval and privacy rules.

This study characterizes its prevalence and associations with recanalization. Methods Consecutive thrombectomy cases at two high-volume stroke centers (January 2019-December 2025) were retrospectively reviewed. Stored fluoroscopy and angiograms were assessed for passive advancement and final catheter tip position at arterial branch points. Per-pass and per-vessel mixed-effects multivariable regression identified independent associations with eTICI score, first-pass effect (FPE, eTICI ≥2c-3), modified FPE (mFPE, eTICI ≥2b-3), secondary occlusion, and complications. Results Of 1,824 passes in 995 consecutive patients (1,027 occlusions), 733 had imaging adequate for passive advancement assessment. Passive advancement occurred in 525/733 passes (71.6%), including to an arterial branch in 306/733 (41.7%). On per-pass analysis, passive advancement was associated with higher eTICI ≥2b-3 (71.0% vs 19.7%) and eTICI ≥2c-3 (57.5% vs 10.1%); advancement to a branch amplified these associations (90.5% vs 32.1% and 79.4% vs 18.7%; all p 87%. Passive advancement should be a technical objective of aspiration thrombectomy. ### Competing Interest Statement MDA: Consulting and equity, Certus Critical Care; Consulting and equity, Route 92 Medical; Consulting, Stryker; Consulting, Medtronic; Consulting, J Consulting and equity, Piraeus Medical; Equity, Galaxy Therapeutics; Consulting, RapidAI; Travel and Consulting, Penumbra.
