## Executive GIST (TL;DR)
- Early-phase Parkinson’s disease (PD) trials commonly use the **MDS-UPDRS Part III** motor score as the primary endpoint, which often requires large sample sizes or extended (12–18 month) treatment durations because early PD progresses slowly relative to measurement variability. - The authors used placebo-arm summary data from four recent Phase 2 trials (SPARK, PASADENA, PADOVA and LUMA) to estimate required sample sizes across a range of assumed treatment effects on **MDS-UPDRS Part III** progression. - They compared sample size estimates for MDS-UPDRS Part III with estimates for smartphone-based sensor-derived **Motor Functional Composite (MFC)** endpoints using a responsiveness-based approach. - Two composite endpoints were evaluated: **MFC 1** (three active assessments: 6-minute walk, U-turn, Hand Pronation-Supination) and **MFC 2** (MFC 1 plus 60-second Typing). - Using the MFC endpoints in a one-year trial was projected to reduce required sample size by more than half compared with MDS-UPDRS Part III, assuming equivalent proportional treatment effects. - Calculations were implemented in a publicly accessible, interactive sample size calculator application hosted by the authors. - All authors are employees or contractors of Indivi AG, which funded the work and developed the MFC endpoints and the sample size application; this competing interest is disclosed. - The analysis used publicly available aggregate summary statistics from the cited trials; no individual participant data were accessed and no new data were collected. All extracted values are reported in the manuscript tables and supplementary material. - Data sources cited include ClinicalTrials.gov disclosures and published reports from NEJM, npj Digital Medicine, Lancet, and a conference poster; full provenance is listed in the manuscript's Data Availability statement.

This is due to the rather slow rate of early PD progression relative to the variability of the MDS-UPDRS Part III assessments. Using placebo-arm data from four recent large Phase 2 trials of early-stage PD (SPARK, PASADENA, PADOVA and LUMA), the sample size needed to detect a range of assumed treatment effects on MDS-UPDRS Part III progression was estimated. The trial sample size estimates obtained for MDS-UPDRS Part III and for smartphone-based sensor-derived motor functional composite (MFC) endpoints were compared using an approach based on relative responsiveness to disease progression. The calculations were also implemented in an interactive, publicly accessible application. Powering a one-year trial using the MFC1 endpoint, derived from three active assessments (6-minute walk, U-turn and Hand Pronation-Supination), or the MFC2, which adds 60-Second Typing, was projected to reduce the required sample size by more than half relative to the MDS-UPDRS Part III, assuming equivalent proportional treatment effects. ### Competing Interest Statement Funding: this work was supported by Indivi AG. All authors are employees or contractors of Indivi AG, which developed the Motor Functional Composites (MFC1 and MFC2) and the sample size calculator application evaluated in this paper. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
