## Executive GIST (TL;DR)
- The investigators performed a blinded external validation of Ataraxis Breast **CTX**, an AI model integrating digitized **H&E** histology and clinical variables, using archived samples from the phase III SWOG-8814 trial. - The analytic cohort included 677 patients with digitized H&E slides; SWOG-8814 data were not used to train or tune CTX and thresholds were locked prior to evaluation. - Two CTX outputs were assessed: a histology-only score and a multi-modal score combining pathology features with clinical data. - The primary endpoint matched the trial primary: disease-free survival (DFS); predictions were generated blinded to outcomes and provided to SWOG for validation. - The histology-only score associated with DFS; investigators reported an effect estimate indicating worse DFS with higher score and added prognostic information beyond clinical variables (statistical significance reported). - The multi-modal CTX showed a stronger association with DFS and higher discrimination (reported C-index 0.66 for multi-modal versus 0.59 for histology-only). - Exploratory treatment-by-biomarker analyses modeled CTX continuously and produced a suggestive interaction for chemotherapy benefit (reported HR 0.82, p = 0.053 for CET versus ET). - Numerical differences in chemotherapy effect were observed between CTX-defined high-benefit and low-benefit subgroups but confidence intervals crossed unity in both groups. - Data are not publicly available; access to SWOG8814 data requires direct request to SWOG. - The study team and funders call for further prospective validation of CTX as a tissue-sparing prognostic and predictive tool; the preprint has not been peer reviewed.

Genomic assays identify patients likely to benefit from chemotherapy but are expensive and tissue consuming. AI-enabled biomarkers may provide scalable alternatives. We performed a blinded external evaluation of Ataraxis Breast CTX, a causal multi-modal AI model that integrates H&E images with clinical variables, in the phase III SWOG-8814 trial. Methods The analytic cohort comprised 677 patients (46% of the 1477 enrolled) with digitized H&E slides. Two CTX scores were evaluated: a histology-only score and a multi-modal score integrating pathology features with clinical variables. CTX weights and thresholds were locked before evaluation and SWOG-8814 data were not used for retraining. Predictions were generated blinded to outcomes and provided to SWOG for validation. The primary endpoint was disease-free survival (DFS), matching the primary endpoint of the trial. Findings The histology-only score was prognostic for DFS (HR = 1.36, 95% CI = 1.22-1.51) and added prognostic information beyond clinical variables alone (p The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
