## Executive GIST (TL;DR)
- This single-centre retrospective cohort included 413 operable breast cancer patients treated with **neoadjuvant chemotherapy (NACT)** between January 2013 and November 2020. - Clinical staging spanned cT1–cT3 and cN0–cN3; median age was 51. The source did not report additional demographic breakdowns. - Pathological concordance between breast and axilla was 81%; both breast and axillary pathological complete response (**breast pCR** and **axillary pCR**) occurred in 22.2% of patients, while concurrent residual disease occurred in 58.8%. - In clinically node-negative (cN0) patients, ypN0 rates were higher when the breast achieved pCR (97.1% vs 80.8%, p<0.001), per the authors. - Among clinically node-positive (cN+) cases, axillary pCR was more likely when breast pCR was present; only 16.5% achieved axillary pCR with residual breast disease. - Logistic regression identified breast pCR as the strongest predictor of axillary pCR (OR=11.6, p<0.001) according to the report. - Axillary pCR rates varied by subtype: highest in ER-/**HER2+** (66.7%) and **triple-negative** (46.9%) tumours; the source provided these subtype-specific rates. - Residual axillary disease was independently associated with worse breast cancer-specific survival (HR=7.13, p=0.006) and overall survival (HR=3.32, p=0.029), per Cox regression reported by the authors. - Kaplan–Meier plots in the source indicated a trend toward poorer survival with axillary residual disease, even among patients with breast pCR. - The authors conclude these findings support selective axillary de-escalation strategies guided by post-NACT assessment, but they emphasise this is a limited series and that patient selection requires careful evaluation. - The study received a waiver from the IRB as a service evaluation/clinical audit; data are available on request. The source did not provide detailed treatment regimens, cycle numbers, or radiological response criteria beyond what is reported above.

Understanding differential response is crucial for de-escalation strategies. Methods: This single university hospital retrospective study analysed operable breast cancers (cT1-cT3, cN0-cN3) treated with NACT (January 2013-November 2020). Two-tailed Fishers exact test or the Chi-squared test (for 2x2 comparisons) analysed concordance between breast and axillary pathological responses; Cox proportional hazards regression ascertained predictors of distant recurrence and survival; and logistic regression ascertained predictors of complete axillary response. Results: Among 413 patients (cN0=229; cN+=184) with a median age of 51, pathological response concordance between the breast and axilla was 81% (both pCR, 22.2%; concurrent residual disease, 58.8%). The cN0 group had a significantly higher ypN0 rate in breast pCR versus non-pCR (97.1% vs 80.8%, p The Chan Zuckerberg Initiative, Cold Spring Harbor Laboratory, the Sergey Brin Family Foundation, California Institute of Technology, Centre National de la Recherche Scientifique, Fred Hutchinson Cancer Center, Imperial College London, Massachusetts Institute of Technology, Stanford University, The University of Edinburgh, University of Washington, and Vrije Universiteit Amsterdam.
