- This preprint used two-sample **Mendelian randomization** (MR) to test whether genetic liability to **cannabis use** causally influences site-specific cancers in people of European ancestry. - Investigators selected 22 independent genome-wide significant single-nucleotide variants associated with cannabis use (p < 5 × 10⁻⁸) from a large GWAS meta-analysis. - Outcome summary statistics for site-specific cancers were obtained from FinnGen (individuals of European ancestry). - Primary MR analyses used inverse-variance weighted (IVW) and multivariable IVW approaches; the multivariable models adjusted for **cigarette smoking**. - In multivariable IVW models adjusted for smoking, genetic liability to cannabis use was associated with higher odds of **esophageal cancer** (OR 1.74, 95% CI 1.29–2.15; p = 0.013) and **lung cancer** (OR 1.35, 95% CI 1.13–1.58; p = 0.009). - Conversely, genetic liability to cannabis use showed a protective association with **pancreatic cancer** (OR 0.77, 95% CI 0.57–0.91; p = 0.032) in the FinnGen sample. - Sensitivity analyses (simple median, weighted median, MR-Egger, and MR-PRESSO) found no evidence of horizontal pleiotropy affecting the cannabis–cancer associations. - The study used publicly available GWAS summary statistics; data supporting the findings are available via FinnGen. - The report is a preprint that has not been peer reviewed; the authors declare no competing interests. Epidemiological evidence on the association between cannabis use and cancer is inconsistent and conflicting.
