## Executive GIST (TL;DR)
- Antibody–drug conjugates (**ADCs**) were developed for targeted cytotoxic delivery but also exert broad **immunomodulatory** effects beyond direct cell kill. - Immunological actions derive from a combination of **antibody-driven Fc functions** (for example, antibody-dependent cell-mediated cytotoxicity) and **payload-induced immunogenic stress**, which together enhance antigen release and dendritic cell activation. - Key mechanisms include **immunogenic cell death**, tumour-associated antigen release, activation of innate immune effectors and reprogramming of the tumour microenvironment to support adaptive T cell responses. - Preclinical and translational data support that ADCs can link tumour antigen recognition to both innate and adaptive immune activation within the tumour microenvironment. - Clinical development has increasingly explored combinations of ADCs with **immune checkpoint inhibitors (ICIs)** and other immune-stimulating agents to convert immunologically ‘cold’ tumours into inflamed, responsive phenotypes. - ADC–ICI combinations have delivered practice-changing benefits in selected indications, but efficacy is inconsistent across settings; the authors note that **biomarker-informed development** will likely be needed. - Next-generation ADC strategies emphasise immune-oriented engineering: bispecific constructs, microenvironment-directed targeting, novel or dual payloads, and the use of immunostimulant payloads to amplify antitumour immunity while keeping target selectivity. - Figures in the article summarise (1) immunological mechanisms of ADC action and (2) innovative strategies to enhance immune modulation by ADCs. - The review frames ADCs not only as targeted cytotoxic agents but as modulators of tumour–immune interactions, with implications for both monotherapy and combination regimens. - The source is a subscription preview; full article access and additional experimental/clinical details are available in the full text and referenced studies.

Emerging evidence indicates that the activity of ADCs extends beyond direct cytotoxicity to encompass a coordinated spectrum of immunomodulatory effects, arising from both antibody-mediated functions and payload-induced cellular stress. These mechanisms include immunogenic cell death and subsequent tumour-associated antigen release, antibody-dependent cell-mediated cytotoxicity, dendritic cell activation and tumour microenvironment reprogramming. On the basis of this biological foundation, ADCs have been increasingly explored in combination with immune checkpoint inhibitors and also with other immune-stimulating agents. Furthermore, novel ADC designs with enhanced immunological activity are emerging. In this Review, we examine the direct and indirect immunological effects underpinning the clinical activity of ADCs, appraise the clinical evidence supporting the combination of ADCs with immune checkpoint inhibitors and highlight innovative strategies to further optimize the immunogenic potential of ADCs, including antibody engineering, targeting of the tumour microenvironment, novel combinations and the use of immunostimulant payloads or bispecific antibodies. This evolving paradigm positions ADCs not merely as targeted cytotoxic agents, but rather as modulators of tumour–immune interactions, with implications for the activity of both monotherapy and combination regimens. ## Key points * Preclinical and translational evidence indicates that antibody–drug conjugates (ADCs) have antitumour activity that extends beyond the targeted delivery of a cytotoxic agent and can include reshaping antitumour immunity, thus linking tumour antigen recognition with activation of innate and/or adaptive immunity within the tumour microenvironment. * The immunological effects of ADCs arise from the interplay between antibody-driven Fc-mediated functions and payload-induced immunogenic stress, collectively leading to enhanced antigen release, dendritic cell activation and the establishment of sustained T cell responses. * ADC–immune checkpoint inhibitor (ICI) combinations have shown practice-changing levels of benefit in selected indications, although efficacy is not universal and biomarker-informed development will probably be required. * Emerging clinical evidence supports combining ADCs with ICIs and next-generation immunotherapies, with various ongoing trials exploring these combinations as methods to convert immunologically ‘cold’ tumours into responsive, inflamed phenotypes. * The next generation of ADC design is shifting towards immune-oriented engineering, including bispecific constructs, microenvironment-directed targeting and novel or dual payloads, designed to amplify immune activation while preserving therapeutic selectivity.
