## Executive GIST (TL;DR)
- The authors performed an **LLM-assisted evidence audit** to evaluate whether **late-stage cancer incidence** is a reliable earlier endpoint for cancer-screening trials, and whether its trial-level association with cancer-specific mortality is robust to added source-verified evidence. - They reconstructed the PubMed corpus underlying a prior 41-comparison review and used Gemini 3.1 Pro Preview only to prioritize reports for blinded human reassessment; all final eligibility, linkage, harmonization, and verification were done by human reviewers. - From 1,209 candidate records, 996 PDFs were assessed. Thirty-three reports not present in the source review were prioritized; of these, 26 were eligible, representing 18 trials, and 8 provided comparisons compatible with the original dataset. - Adding the 8 compatible comparisons increased the comparison set from 41 to 49 and reduced the overall unweighted Pearson correlation between late-stage incidence and cancer-specific mortality from 0.73 (95% CI 0.55–0.85) to 0.59 (95% CI 0.37–0.75). - Updated cancer-specific correlations were reported as 0.49 (95% CI −0.26 to 0.87) for breast cancer, −0.23 (95% CI −0.71 to 0.40) for colorectal cancer, and 0.83 (95% CI 0.54 to 0.95) for lung cancer; a sparse-event comparison materially influenced the colorectal estimate. - The authors conclude that the overall association is sensitive to the composition of evidence and that stability varies by cancer type; they recommend evaluating late-stage incidence by cancer type with prespecified sensitivity analyses for evidence selection and endpoint harmonization. - They emphasize that model-assisted prioritization cannot replace human eligibility review, trial reconciliation, and source verification. Data and code from the audit are available in an open repository at the Open Science Framework provided in the source. - The authors declared no competing interests and reported relevant funding sources and ethical compliance as noted in the source article.

We evaluated the robustness of this association to source-verified additions. Methods We reconstructed the PubMed corpus underlying a 41-comparison review. Gemini 3.1 Pro Preview was used only to prioritize reports for blinded human reassessment. Reviewers determined eligibility, linked reports from the same trial, harmonized endpoints, and verified comparison-level data. We recalculated unweighted Pearson correlations overall and by cancer type after adding earliest-compatible trial comparisons. Results Among 1209 candidate records, 996 PDFs were assessed. Thirty-three reports absent from the source review were prioritized; 26 were eligible, representing 18 trials, and 8 provided compatible comparisons. Adding these comparisons increased the dataset from 41 to 49 and attenuated the overall correlation from 0.73 (95% confidence interval [CI] = 0.55 to 0.85) to 0.59 (95% CI = 0.37 to 0.75). Updated correlations were 0.49 (95% CI = -0.26 to 0.87) for breast, -0.23 (95% CI = -0.71 to 0.40) for colorectal, and 0.83 (95% CI = 0.54 to 0.95) for lung cancer. One sparse-event comparison influenced the colorectal estimate. Conclusions The overall association was sensitive to evidence composition, and cancer-specific stability varied. Late-stage incidence should be evaluated by cancer type and with prespecified sensitivity analyses for evidence selection and endpoint definitions. Model-assisted prioritization cannot replace human eligibility review, trial reconciliation, and source verification. ### Competing Interest Statement The authors have declared no competing interest. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained. Yes I confirm that all necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived, and that any patient/participant/sample identifiers included were not known to anyone (e.g., hospital staff, patients or participants themselves) outside the research group so cannot be used to identify individuals. Yes I understand that all clinical trials and any other prospective interventional studies must be registered with an ICMJE-approved registry, such as ClinicalTrials.gov. I confirm that any such study reported in the manuscript has been registered and the trial registration ID is provided (note: if posting a prospective study registered retrospectively, please provide a statement in the trial ID field explaining why the study was not registered in advance). Yes I have followed all appropriate research reporting guidelines, such as any relevant EQUATOR Network research reporting checklist(s) and other pertinent material, if applicable.
