## Executive GIST (TL;DR)
- Head and neck squamous cell carcinoma (**HNSCC**) most commonly arises from mucosal squamous epithelium of the oral cavity, pharynx and larynx; multimodal therapy is standard. - Primary oral cavity tumours are managed with surgical resection followed by adjuvant **cisplatin**-based chemoradiotherapy, whereas pharyngeal and laryngeal disease often receive definitive concurrent chemoradiotherapy. - For HPV-negative patients who cannot tolerate platinum, the epidermal growth factor receptor monoclonal antibody **cetuximab** is given with radiotherapy as an alternative systemic agent. - Immune checkpoint inhibitors (**ICIs**) have changed systemic treatment for recurrent/metastatic disease, producing durable responses and a survival advantage in that setting. - Approximately 65–70% of patients present with stage III–IVA **locally advanced** disease, a group with major therapeutic and quality-of-life challenges. - The stated goals of neoadjuvant therapy are tumor downstaging, eradication of occult micrometastatic disease, and improved locoregional control before definitive surgery or radiotherapy — with organ preservation as a driving objective. - Contemporary neoadjuvant regimens commonly combine taxane, platinum and 5‑fluorouracil induction chemotherapy with radiotherapy; this combination is described as a current clinical backbone. - Early-phase clinical trials have investigated adding **ICIs** into neoadjuvant protocols; the review emphasizes these immunotherapy-based paradigms as emerging approaches to improving oncologic and functional outcomes. - The source is a narrative review summarizing the evolution of neoadjuvant therapy; specific trial doses, sample sizes, endpoints and numerical outcomes were not reported in the abstract.

A multimodal standard of care is applied for case management. Primary oral cavity cancers are treated with resection followed by adjuvant cisplatin‑based chemoradiotherapy (CRT), and definitive concurrent CRT is applied for pharyngeal or laryngeal disease. In human papillomavirus‑negative patients who are medically unfit for platinum treatment, the epidermal growth factor receptor‑directed monoclonal antibody cetuximab is administered concomitantly with radiotherapy (RT). More recently, immune checkpoint inhibitors (ICIs) have redefined systemic therapy for recurrent/metastatic HNSCC, yielding durable responses and a survival benefit. In total, 65‑70% of patients present with stage III‑IVA locally advanced (LA) disease, a scenario associated with therapeutic challenges and a marked deterioration in the quality of life. Neoadjuvant therapy is designed to downstage the primary tumor, eradicate occult micrometastatic disease and enhance locoregional control before definitive surgery or RT, thus offering a rational strategy for organ preservation. Contemporary regimens integrate taxane, platinum and 5‑fluorouracil induction chemotherapy with RT, and early‑phase trials have explored the feasibility and efficacy of incorporating ICIs into neoadjuvant protocols. The present review summarizes the evolution of neoadjuvant therapy for LA HNSCC, with particular emphasis on emerging immunotherapy‑based paradigms and their potential to improve oncological and functional outcomes. **Keywords:** chemotherapy; head and neck squamous cell carcinoma; immunotherapy; locally advanced; neoadjuvant therapy; radiotherapy.
