## Executive GIST (TL;DR)
- A recent reassessment evaluated whether adding chemotherapy to immune checkpoint inhibitor (ICI) therapy improves outcomes for advanced non–small-cell lung cancer (NSCLC) with **PD-L1 TPS ≥ 50%**. The analysis used trials included in a prior meta-analysis (search date August 3, 2025). - The authors extracted hazard ratios (HRs) for overall survival (OS) and progression-free survival (PFS) from 24 phase 3 randomized trials reported in the original meta-analysis. - Two analytic frameworks were applied: within-agent comparisons (same ICI used in both chemoimmunotherapy and ICI monotherapy arms) and across-agent comparisons (different ICIs used across strategies). - Within-agent comparisons covered 4 ICIs across 13 trials (N = 3,252). The pooled ratio of HRs (RHR = HRchemoimmuno/HRmono) was 0.94 (95% CI, 0.78–1.13) for OS (P = .48; I2 = 0.0%) and 0.85 (95% CI, 0.68–1.06) for PFS (P = .14; I2 = 0.0%), indicating no statistically significant benefit of adding chemotherapy. - Across-agent comparisons included 7 ICIs across 11 trials (N = 2,231) and favored chemoimmunotherapy: RHR 0.68 (95% CI, 0.50–0.92; P = .01) for OS and 0.46 (95% CI, 0.37–0.58; P < .001) for PFS. - A sensitivity analysis limited to National Comprehensive Cancer Network (NCCN)–recommended regimens produced a pooled RHR for OS of 1.02 (95% CI, 0.81–1.28; P = .87), consistent with no OS benefit from adding chemotherapy. - The authors conclude that the apparent survival advantage reported in the prior meta-analysis is likely driven by heterogeneity across different ICIs rather than a true chemotherapy effect; findings support **ICI monotherapy** as a standard first-line option for PD-L1 TPS ≥ 50% disease and highlight the need for agent-level stratification in cross-trial comparisons. - The study used aggregate published trial HRs and reports data availability on request. However, whether this benefit reflects chemotherapy effect or ICI heterogeneity remains unclear. OBJECTIVE: To reassess the survival benefit of adding chemotherapy to ICI monotherapy using agent-stratified comparisons anchored to chemotherapy. DATA SOURCES: The 24 phase 3 randomized clinical trials included in the original meta-analysis (search date, August 3, 2025). DATA EXTRACTION AND SYNTHESIS: Hazard ratios (HRs) for OS and progression-free survival (PFS) were extracted from each trial in the original meta-analysis. Two analytic frameworks were used: within-agent comparisons (same ICI in both chemoimmunotherapy and monotherapy) and across-agent comparisons (ICI in one treatment strategy only). For within-agent comparisons, a two-stage random-effects meta-analysis was conducted.
