## Executive GIST (TL;DR)
- The study examined changes in the mean number of **blood transfusions** for **thoracic surgery** at The Ottawa Hospital after regionalization of lung cancer surgery. - Investigators used aggregate data scraped from Shehata et al. (2014) titled "Changing trends in blood transfusion: an analysis of 244,013 hospitalizations" to estimate effects on the log mean number of transfusions. - The analysis compared thoracic surgeons (lung cancer operators) to other surgical specialties, which served as control groups in the model. - Data indicated the average number of transfusions rose significantly among thoracic surgeons after a new cancer centre opened at The Ottawa Hospital in December 2009, while control specialties did not show a parallel increase. - The authors framed results using a modified population-level treatment-threshold model to explain observed changes in transfusion frequency. - The study team argued the primary driver was an influx of less experienced thoracic surgeons to The Ottawa Hospital combined with closure of thoracic units at smaller hospitals as part of regionalization. - The paper is a medRxiv preprint; it has not been peer reviewed and the authors state that all relevant ethical approvals were obtained. - All data from the study are available on reasonable request to the authors; methodological specifics beyond aggregate-source use were limited in the report.

Using aggregate data scraped from the paper ''Changing trends in blood transfusion: an analysis of 244,013 hospitalizations'' by Shehata et al. (2014) we estimated the effect regionalization on the log mean number of blood transfusions among thoracic surgeons (who treat lung cancer), utilizing other surgical specialties as controls. We found that the average number of blood transfusions increased significantly among thoracic surgeons following the opening of a new cancer care centre at the hospital in December 2009, while control specialties showed no parallel change. Contextualizing these results in a modified ''population level'' treatment threshold model, it is argued that the main driver of this increase in blood transfusions among thoracic surgeons was due to the influx of less skilled thoracic surgeons to The Ottawa Hospital, and the closing of thoracic surgery units at smaller hospitals which was part of the broader regionalization process. ### Competing Interest Statement This paper was prepared for the Georgetown Blood and Plasma Research Group Fellowship. ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
